Job Summary:

The Site Services coordinator for the Parkmoor project supports up to 79 residents in a permanent supportive housing unit through case management, providing resources, attending ongoing meetings, organizing life skills groups, planning monthly events for participants, resolving conflict issues, and teaching daily life skills. The Site Services coordinator will work closely with case managers and coordinate with property management staff.

Essential Duties/Responsibilities:

Client Assessment And Planning

  • Work together with case manager, residents, and property manager.
  • Ensure all residents meet eligibility requirements appropriate for the units they are being offered.
  • Ensure compliance with policy and procedure, budget, contract outcomes, and requirements.
  • Conduct assessments to understand each family's unique needs and situation.
  • Collaborate with families to develop individualized case plans that address their goals for housing stability, employment, and self-sufficiency.
  • Maintain accurate records and reports.

Resource Connection And Advocacy

  • Link families with essential resources like job training, education, healthcare, legal aid, and permanent housing assistance.
  • Advocate for families in their interactions with government agencies and service providers.

Support And Empowerment

  • Provide ongoing support and guidance to families as they work towards their goals.
  • Help families develop coping mechanisms, budgeting skills, and life skills necessary for independent living.

Monitoring And Evaluation

  • Monitor families' progress towards achieving their goals.
  • Document activities and outcomes.
  • Analyze data to identify areas for improvement in services.

Collaboration And Communication

  • Collaborate with other staff, the property manager, outside agencies, and community partners to ensure comprehensive services for families.
  • Communicate effectively with families, respecting their privacy and confidentiality.

Additional Responsibilities

  • Participate in professional development opportunities.
  • Attend agency and other program meetings and trainings as appropriate.
  • Participate in agency Program Quality Improvement activities and initiatives.
  • Participate in agency Safety initiatives and proactively report safety issues to management immediately.
  • Regular attendance is a requirement of the position.
  • Other duties may be assigned.
Qualifications

Education and Experience:

  • (B.A.) in Social Work, Sociology, Psychology, or related field plus 2+ years of relevant case management experience and/or training; or equivalent combination of education and experience. Preferred experience working with homeless clients and or in a shelter setting.

Required Skills/Abilities:

  • Driving is a requirement of the job (valid California driver's license with access to a personal vehicle that can be used for company business and current personal automobile insurance).
  • Current CPR and First Aid Certification required (or complete within 30 days of hire).
  • Fingerprinting/background check as required by the Department of Justice and/or Title XXII regulations. TB test required.
  • Ability to work with basic mathematical concepts as required for reporting. 
  • Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.
  • Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
  • To perform this job successfully, an individual should have knowledge of Internet software; Spreadsheet software and Word Processing software. Knowledge of Google Suite desired. Internet research skills are a plus.

Salary Range:

$24-$27 per hour
