Charge Nurse LVN

CC Young, a Continuing Care Retirement Community and a not-for-profit leader in Senior Housing and Health Care, has an immediate opening for a seasoned, high-energy full-time Licensed Vocational Nurse (LVN) or Registered Nurse (RN) for our Health Center. Our ideal candidate will be responsible for providing skilled nursing care for clients on our skilled nursing units.

Available Shift: Monday- Friday 7:00am- 3:00pm

The job duties for this position include, but are not limited to:

• Provides skilled nursing care to clients in our Health Center.
• Coordinates care with interdisciplinary team to ensure optimal care for and benefit to each client.
• Communicates with clients, family members, interdisciplinary team members and management regarding client status.
• Prepares and administers medications and treatment as ordered by physician.
• Assists with and participates in development and implementation of written care plans.
• Ensures the accurate and timely documentation of all client treatment, medication and other pertinent information.
• Follows established procedure for charting and incident reporting.
• Follows company procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Documents any change in patient's conditions including but not limited to nutritional problems, infections, skin conditions, weight loss or any other noticeable changes.
• Makes rounds with physicians when necessary.
• Performs other duties as assigned.

Our new team member will possess the following minimum qualifications:

• Possession of a valid license as a Licensed Vocational Nurse (LVN) or Registered Nurse (RN) in good standing within the State of Texas.
• Patient Care experience is highly preferred.

Pay Range: $26.00- $37.00
