Housing Case Manager - Outreach
Position Summary
The Housing Case Manager helps participants who have experienced homelessness flourish. The Housing Case Manager (HCM) serves as a Housing Resource Specialist (HRS), trained by the All Chicago Continuum of Care, and is responsible for providing case management services that lead clients in transition from homelessness to housing and other more stable living situations. The HCM will remain in compliance with funding guidelines and agency policies and procedures; for providing follow-up services for outreach clients in the Engagement Center; assisting the Supportive Services and Street Outreach teams with effective record keeping, and supporting the Street Outreach program development initiatives. The Housing Case Manager will lead the systems navigation process to support individuals experiencing unsheltered homelessness to access basic needs, health-related services, and stabilization support as they transition from homelessness to housing. The Housing Case Manager will ensure that the outcomes listed by the U.S. Housing and Urban Development (HUD) for the Street Outreach Program are met.
The Housing Case Manager reports to the Associate Program Director - Engagement & Outreach.
Position Responsibilities
Program Delivery (70%)
- Provide comprehensive housing-related case management services to clients and their families to support effective transition to housing, maintenance or increase of income, and increase in skills and resources
- Complete Coordinated Entry System (CES) skilled assessments as a HRS for individuals currently experiencing homelessness
- Collaboratively develop and update individual service plans with clients, providing encouragement, education, and advocacy toward clients' individual goals
- Provide life skills instruction in formal and informal settings, possibly including, but not limited to financial literacy, education, education, health/wellness education and access, and other skill development as needed
- Lead engagement with supportive housing case managers to assist clients in all documentation needs for housing transition
- Utilize motivational interviewing, trauma-informed care, and harm reduction strategies to support movement toward housing stability, increased skills and income, improved health, and achievement of personal goals
- Provide housing-related case management for outreach clients in the Engagement Center, including but not limited to welcoming clients, running groups, coordinating food and coffee access, helping clients with engaging in and accessing center resources, and facilitating relationships between clients and services by community partners
- Provide crisis intervention as needed
- Connect clients to community resources, complete referrals for appropriate services and document referrals in service plans as indicated by agency policy
- Coordinate and/or provide transportation as appropriate for client
- Assist in the development of linkage agreements with service providers
- Distribute transportation assistance, food assistance, and donated goods in accordance with agency policy
- Conduct follow up with past clients in accordance with HMIS guidelines
- Perform other duties as assigned
Program Administration (30%)
- Document all client contacts and all services provided in progress notes
- Assist Supportive Services staff in record keeping
- Maintain updated paper files in neat and legible condition
- Complete all required paperwork in an accurate and timely manner, including but not limited to: assessments, service plans, contact log, compliance form, and matching gifts form
- Complete timely and accurate data entry into HMIS and any additional database as needed according to agency policy
- Complete closing documentation including final service plans, closing form and HMIS
Qualifications
- Bachelor's degree or related experience in social work/psychology/related human service field
- One year of case management experience (preferred)
- Knowledge of issues surrounding homelessness, mental illness, substance use and trauma
- Knowledge of housing systems, local context of housing stock, and pathways to housing
- Strong verbal and written communication skills
- Excellent interpersonal skills with participants, colleagues, and community partners
- Ability to work effectively with persons who may be ill, disabled, emotionally dysregulated and/or agitated through crisis intervention and de-escalation techniques
- Ability to show empathy and remain nonjudgmental
- Cultural responsivity with respect for diversity
- Ability to work flexible hours. Shifts will include evening and weekend hours
- Bilingual in Spanish and English a plus
- Proficient in use and knowledge of Google Suite of products and general office equipment
- Ability to make independent judgments and escalate needs to appropriate support staff
- Ability to manage multiple tasks
- Valid Driver's License, Auto Insurance commensurate with personnel policy
- Ability to maintain confidentiality of participant information
- Commitment to the mission of ReVive Center
Compensation
- Starting annual salary $43, ,000, paid twice a month
- Health Care, Dental, and 403B savings plan with a 3% agency match

Working Conditions and Physical Requirements
- Travel between client and assigned location(s)
- Work is normally performed on the street or in the engagement center with some time in an office or client residences
- Exposure to all varieties of Chicago weather
- Able to conduct independent home visits
- Physical activity including standing, walking, bending, climbing stairs, lifting (less than 10 pounds)
- Functional hearing, vision, manual dexterity and hand-eye coordination or effective accommodations in place to perform job functions
- Effectively work on teams with interprofessional collaboration
