Job Description

We are aggressively seeking an Ambulatory Case Manager RN to join a large Healthcare organization. As the year 2016 is in full swing, this organization is very busy, and they need YOUR help.

Daily Responsibilities: 

  • Conduct telephonic prospective, concurrent and retrospective review of active patient care.
  • Coordinates treatment plans and conduct transitional care calls.
  • Prioritizes patient care needs.
  • Developed care plans with time frames and obtainable outcomes.
  • Reevaluate care plans, time frames and outcomes.
Qualifications

Requirements as Ambulatory Case Manager:

  • Bachelor's degree in Nursing
  • Active California RN license in good standing
  • 3+ years of Clinical Nursing with 1 of those years being within an Outpatient setting
Additional Information

Pay Rate: $38/hr. - $41/hr.

Hours for this Position:

  • Shift: Monday-Friday 8-5

Ambulatory Care Nurses take care of patients in environments outside of hospitals. Their primary focus lies in pain management and general health education for patients with chronic injuries or illness.
