Our client, a mission-driven nonprofit health organization providing comprehensive care to underserved New Yorkers. We're seeking a skilled and compassionate Licensed Clinical Social Worker (LCSW) to join our East Harlem team.

What You'll Do

  • Provide clinical social work services, including assessments, counseling, and psychotherapy in an inpatient care setting.
  • Collaborate with medical and behavioral health providers to develop and implement patient care plans.
  • Coordinate referrals, entitlement support, and community services to address patient needs.
  • Support HIV prevention efforts through adherence support, data collection, and follow-up.
  • Document services in our EHR and ensure compliance with HIPAA and organizational policies.
  • Assist in crisis situations involving child abuse, domestic violence, or elder abuse.
  • Contribute to quality improvement initiatives and care coordination meetings.

What Our Client is Looking For

  • LCSW licensed in NYS with 3+ years post-MSW experience, including 1+ year in an outpatient/ambulatory setting.
  • Experience with chemical dependency, MAT, and HIV services preferred.
  • Strong interpersonal, organizational, and communication skills.
  • Culturally competent and comfortable working with diverse communities.
  • Bilingual (English/Spanish) strongly preferred.
  • Proficient in Microsoft Word and EHR systems.
