About the Position

Join our established Assertive Community Treatment (ACT) team as a Mental Health Clinician in the Bronx. This community-based role combines meaningful fieldwork with collaborative team support, offering smaller caseloads that allow you to build lasting therapeutic relationships and witness real recovery outcomes.

Location: Bronx, NY (Community-Based with On-Site Team Collaboration)

Schedule: Monday–Friday, 8:00 AM–4:00 PM or 9:00 AM–5:00 PM

What You'll Do

As a Mental Health Clinician on our ACT Team, you'll provide intensive, recovery-focused support to adults living with serious mental illness. Your day begins with a team huddle, followed by 3-4 scheduled community visits to deliver therapeutic services in the field.

Core Responsibilities:

Clinical Services:

  • Provide individual, group, and family therapy using evidence-based practices (CBT, Motivational Interviewing, trauma-informed care)
  • Conduct comprehensive assessments, screenings, and risk evaluations
  • Develop person-centered care plans, WRAP plans, and relapse prevention strategies
  • Deliver crisis intervention and stabilization services

Community-Based Care:

  • Complete 3-4 community/home visits daily to pre-vetted locations in the Bronx
  • Support clients following psychiatric hospitalizations (CTI-informed approach)
  • Coordinate care with hospitals, primary care providers, and community partners
  • Assist with service navigation, scheduling, and transportation coordination

Documentation & Teamwork:

  • Maintain thorough documentation in electronic health records (EHR)
  • Participate in weekly team meetings, case conferences, and clinical training
  • Collaborate within a multidisciplinary team environment
  • Contribute to a positive, recovery-oriented team culture
  • Participate in on-call rotation every 6-8 weeks (minimal call volume)

QualificationsRequired:

  • Master's degree in Social Work, Mental Health Counseling, Psychology, or related behavioral health field
  • Minimum 1 year of mental health or human services experience
  • Strong clinical skills in assessment, crisis intervention, and therapeutic modalities (MI, CBT, trauma-informed care)
  • Comfortable conducting community-based home visits in the Bronx
  • Ability to work independently and collaboratively within a multidisciplinary team
  • Excellent documentation and time management skills

Preferred:

  • LMSW, LMHC, or LCSW license
  • Experience with ACT, Intensive Case Management (ICM), or community mental health programs
  • Knowledge of evidence-based practices for serious mental illness
  • Bilingual abilities (Spanish/English) a plus
