Position Overview

Under general supervision from supervisor or administrator; provides intensive case planning services to clients; investigates allegations of abuse, neglect, self-neglect and exploitation. Maintains caseload of individuals and/or families. Participates in defining, establishing and implementing supportive services goals and plans for individual and/or family. Prepares, compiles and maintains social histories, case records, treatment data, supportive material and any reports or forms required by federal regulations, ORC statute or executive order. Establishes and maintains working relationship with community agencies (e.g., coordinates services, attends conferences and workshops, speaks to civic associations); makes court appearances as necessary.

Summary of Job Duties

Recognizes and evaluates if elder abuse (neglect, self-neglect, physical/emotional abuse and exploitation) is occurring. Conduct home visits and evaluate for emergent situations to include possible client removal. Participates in defining, establishing and implementing individual supportive services and plans. Collects, analyzes and interprets reliable and valid information; assesses progress of treatment plan and re-determines and make arrangements for needed services. Identifies and appraises actual and potential resources and services; proposes and develops feasible alternative arrangements and referrals; handles difficult and emergency cases (e.g., crisis intervention); maintains selective cases of alleged abuse, neglect, self-neglect, or exploitation in adult protective services caseloads.

Maintains necessary case records, data snd supportive material; prepares and compiles social histories, discharge summaries, court documents and referrals; completes forms and writes reports as required by law and/or executive order or for research activities; complies with federal and state standards in providing progress notes, case plans and evaluations; prepares any necessary correspondence to families, courts, state and community agencies.

Establishes and maintains working relationship with community partners to include Area Agency on Aging, Legal Aide, senior service agencies and local law enforcement. Participate in case conferences with community health organization providing discharge planning, continuity of services and involvement in patient care; make an assessment of the home to ensure smooth client transition to and from treatment facilities; maintains appropriate contact with courts, making court appearances as necessary; attends conferences and workshops; speaks to civic associations as a department representative. Ensure all cultural issues are addressed in the client case plan, consulting with other knowledgeable staff as needed.

Conduct training of new APS staff and community partners on APS policies and procedures. Assist in the planning and execution of community events involving seniors and APS related issues which includes participation in the multi-disciplinary team (Performs Related Duties as Required)

Minimum Qualifications and Requirements

Master in Social Work, Criminal Justice or related field and one year experience as an Adult Protective Service Specialist 1.

BA/BS in Social Work, Sociology, Psychology and Criminal Justice and at least three (3) years of experience in Adult Protective Services
