Esteem Health is a data-driven Telehealth and mental health company dedicated to providing high-quality, solution-focused care. We are pioneering a two-person team model for our Enhanced Care Management (ECM) program, where the Inbound Lead Care Manager (ILCM) works in tandem with an Outbound Lead Care Manager to provide comprehensive care to a shared caseload of approximately 100 members.
About the Role
As an Inbound Lead Care Manager (ILCM), you will be the anchor of the care team, working primarily from a desk in a highly structured and organized environment. Your day will be focused on implementing the "flight plan" for each member, leveraging electronic communication to guide them through our life support and personal development programs. You will be responsible for the digital touchpoints and strategic planning that ensure members receive continuous, well-documented care.
Key Responsibilities
- Build the daily "Flight Plan": Develop, implement, and manage strategic care plans for members. Your primary focus will be on the proactive and structured elements of care, including scheduling, resource allocation, and program enrollment.
- Electronic Member Engagement: Serve as the primary electronic point of contact for members, connecting with them via phone, email, text, and our digital platform. You will be responsible for pushing for the completion of critical forms (e.g., "Forms C and D") and ensuring all digital communication is documented.
- Connect Members with Resources: Based on the "flight plan," you will be responsible for digitally connecting members with the necessary medical, behavioral health, and community-based services to meet their needs.
- Proactive Planning: Proactively plan next steps for families and individuals on the caseload, ensuring a clear and organized path forward. This includes anticipating future needs and scheduling follow-up activities.
- Documentation & Compliance: Maintain accurate and detailed records in the electronic health record (EHR) system. You are responsible for assuring full compliance with all program guidelines.
- Team Support: Provide direct support to the Outbound Lead Care Manager (OLCM) as needed and be available for emergency home visits or critical in-person issues that arise.
What We're Looking For
- Fluency in Spanish (preferred) & English
- Experience in case management, care coordination, or a related field with a strong focus on structured, desk-based work.
- Familiarity with the CalAIM Enhanced Care Management (ECM) model is a plus.
- Exceptional organizational skills and an ability to manage a large caseload with a high degree of precision and detail.
- Strong communication skills, particularly in an electronic format.
- Passion for serving vulnerable and underserved populations and a commitment to meticulous planning and documentation.
