We are seeking a highly organized, empathetic, and detail-oriented Continuing Care Coordinator to join our team and support clients as they transition through levels of care.

Position Summary

The Continuing Care Coordinator plays a key role in ensuring clients have a safe, structured, and supportive plan for ongoing treatment after completing residential or outpatient programming. This position works closely with clinical staff, case managers, families, and external providers to develop individualized aftercare plans and connect clients with the appropriate resources in the community.

Key Responsibilities

  • Develop comprehensive continuing care/aftercare plans for clients transitioning out of treatment
  • Coordinate referrals to outpatient therapy, psychiatric services, MAT providers, support groups, and sober living environments
  • Maintain regular communication with clients, families, and treatment teams regarding discharge planning
  • Facilitate collaboration between internal clinicians and external providers to ensure continuity of care
  • Provide education on recovery resources, relapse prevention, and long-term support options
  • Document all discharge planning activities in the electronic medical record accurately and in a timely manner
  • Track follow-ups post-discharge and assist clients in accessing recommended services
  • Participate in multidisciplinary team meetings and care coordination discussions
  • Serve as a supportive point of contact for clients and families during the transition process
  • Maintain compliance with all state, federal, and accreditation standards

Qualifications

  • Previous experience in addiction treatment, mental health, case management, or discharge planning required
  • Strong understanding of recovery-oriented systems of care and community resources
  • Excellent communication, interpersonal, and documentation skills
  • Ability to manage multiple priorities and work effectively in a fast-paced clinical environment
  • Compassionate, client-centered approach with strong professional boundaries

Location: Plano, Texas (on site)

Salary: $55,000–$70,000 (based on experience)

Schedule: Full time Monday–Friday (with flexibility as needed)
