The RN Case Manager is responsible for coordinatingcontinuum of care activities for assigned patients and ensuring optimumutilization of resources, service delivery, and compliance with medical regime.

Minimum Requirements

  • Current RN licensure in state practicing
  • At least one year of Case Management experience preferred
  • Current CPR if applicable
  • TB questionnaire, PPD or chest x-ray if applicable
  • Current Health certificate (per contract or stateregulation)
  • Must meet all federal, state and local requirements
  • Must be at least 18 years of age

Registered Nurse (RN) Case Manager – Hospice
Location:
Charlottesville, VA

Schedule:
Monday–Friday, 8:00 AM–4:30 PM | On-call every other weekend

Pay:
$50/hour

Mileage Reimbursement:
$0.70/mile

Position Overview:
We are seeking a compassionate and skilled Registered Nurse to join our hospice team as a Case Manager. This role involves providing care, comfort, and support to terminally ill patients in both home and facility settings across Albemarle County. The RN Case Manager will manage a caseload of approximately 15 patients, conducting 4–5 visits per day, with travel up to one hour from the office.

Key Responsibilities

  • Deliver high-quality hospice care in accordance with nursing standards
  • Make clinical judgments and coordinate patient care with supervision
  • Maintain accountability for patient outcomes and assist in care management
  • Provide emotional support and education to patients and families
  • Document care accurately and timely

About the Role:
This is a field-based position requiring travel to patient homes and facilities. Candidates must be comfortable working independently, managing a dynamic caseload, and providing end-of-life care with empathy and professionalism.
