Erie Family Health Centers, a nationally recognized top workplace with 13 sites in Chicago and suburbs, is looking for a valuable addition to our Providers team The Advanced Practice Provider will provide comprehensive primary health care service to patients in the prevention, diagnosis and treatment of disease. Acts as primary health care provider with the scope of his/her primary care specialty. Performs physical examinations on new and returned clients. Refers patients to specialists as necessary. Documents all patient care activities. Maintain a productivity level necessary to reach mandated annual level of encounters.

  • Compensation is based on each candidate's experience, skills and education within the range identified for the role. Candidates who meet the minimum requirements of the role will start at entry in the range. Any additional skills, experience and education will be reflected in the compensation offered.

Main Duties & Responsibilities

  • Performs history and physical on new/return patients throughout the lifespan.
  • Orders appropriate diagnostic/screening test and reviews test to establish diagnosis; initiates or continues treatment based on findings.
  • Manages acute and chronic illness according to established protocols and/or in collaboration with physicians.
  • Provides follow-up health care providers or community agencies as necessary.
  • Maintains a productivity level necessary to reach mandated annual level of client encounters.

Qualifications

Education

  • M.S. in Physician Assistant Studies, MSN or DNP from accredited nursing school
  • Active PA or APRN license in Illinois
  • Board Certified
  • Active DEA and controlled substance License
  • Active CPR Certification

Experience:

  • Minimum of two years of experience as Family practice and/or primary care
  • Preferred experience serving in FQHC, Community Health Centers or traditionally unserved patient populations
  • Preferred Medical Spanish experience or highly motivated to learn to utilize interpreters for patient encounters in foreign languages

Skills And Knowledge

  • Commitment to increasing number of APRNs working in underserved communities and providing high-quality compassionate patient care.
  • Comfort with providing clear and constructive feedback
  • Energetic, deadline-driven, self-starter
  • Excellent organizational and project management skills
  • Working knowledge of PC using Microsoft products such as Word and Power Point in a Windows operating environment
  • Ability to develop and maintain excellent working relationships with preceptors, partners, clinical staff, managers, and directors.
  • Interest in and comfort with working with diverse patient population
  • Knowledge of or interest in learning medical Spanish
