Work Schedule: Tuesday-Saturday (9:00am-5:00pm)

The Care Coordinator plays a pivotal role in ensuring that patients receive comprehensive, continuous, and high-quality healthcare services. This position involves managing the coordination of care for patients, particularly those with complex or chronic conditions, by bridging communication among healthcare providers, specialists, and community resources.

The Care Coordinator acts as the primary point of contact for patients, facilitating access to necessary services, ensuring understanding of care plans, and advocating for patient needs. He/she supports patients with chronic conditions from a distance, using digital tools to monitor biometric data (like blood pressure, glucose), educate on devices, escalate critical results, coordinate care with providers, and ensure plan adherence, aiming to improve outcomes and reduce hospitalizations through daily virtual check-ins, data analysis, and strong communication.

A successful Care Coordinator will improve patient adherence to care plans, enhance clinical outcomes, reduce hospitalizations and readmissions and increase patient engagement and satisfaction.

Key Responsibilities:
  • Monitor daily patient data (vitals, readings) via RPM/CCM platforms.
  • Serve as the main contact for patients, providing guidance, education, and encouragement.
  • Conduct regular checkins, update care plans, and offer support for selfmanagement.
  • Identify concerning trends or critical alerts in patient data.
  • Promptly escalate issues to the healthcare provider team.
  • Collaborate with doctors, nurses, and other staff for continuity of care.
  • Maintain accurate records in EMR system (eClinicalWorks) and other platforms.
  • Generate reports on patient progress, program metrics, and outcomes.
  • Participate in quality assurance, peer reviews, and workflow improvement.
  • Train new staff on RPM/CCM workflows and platform use.
  • Facilitate effective communication and information exchange among all members of the healthcare team, including external providers and community partners.
  • Schedule and coordinate appointments, tests, and procedures.
  • Maintain accurate and timely documentation of all care coordination activities in the electronic health record (EMR).
  • Serve as the patient advocate, ensuring the patient's preferences, rights, and needs are respected and addressed throughout the care process.
  • Identify and connect patients with appropriate community resources, social services, and support programs (e.g., transportation, financial assistance, food services).
  • Monitor resource utilization and identify opportunities for costeffective care delivery without compromising quality.
  • Assist with insurance authorization and referral processes as needed.
  • Participate in quality improvement initiatives related to patient outcomes, care transitions, and patient satisfaction.
  • Track and report on key performance indicators (KPIs) related to care coordination effectiveness (e.g., readmission rates, screening compliance).
  • Other jobrelated duties as assigned.
Requirements:
  • Active Licensed Practical Nurse (LPN) license State of Illinois
  • BLS certification (required)
  • Minimum 2 years LPN experience
  • Familiarity with RPM platforms, EMR (eClinicalWorks preferred), and digital communication tools.
  • Exceptional communication skills (empathy, active listening), organization, time management, problemsolving, and cultural competency.
  • Previous case management, telehealth, or remote care coordination experience required.
  • Excellent interpersonal and relationshipbuilding skills.
  • Strong organizational and time management abilities with attention to detail.
  • Proficiency in using EMR and RPM/CCM standard office software (e.g., Microsoft Office Suite).
  • Ability to work independently, exercise sound judgment, and manage multiple priorities effectively.
  • Deep understanding of chronic disease management principles and healthcare delivery systems.
  • Demonstrated cultural competence and ability to work effectively with diverse patient populations.

Both locations are moving to 6/days a week:

Halsted (open every day except Sat)

BoY (open every day except Sun)
