Our Physical Therapist Will:

  • Work closely with patients, families, and physicians to provide a personalized care experience by assessing individual's abilities and needs.
  • Seek out and learn new principles and methodologies of care to incorporate in clinical practices.
  • Initiate and revise care plans to meet patient goals.
  • Maintain records and ensure care provided enhances the lives of our patients and meets state and federal guidelines.
  • Build positive relationships throughout the home health team and community.

Our Physical Therapist Has:

  • An active state-required Physical Therapy license.
  • Experience with home health OASIS in order to assist with SOC assessments (preferred).
  • A current CPR/BLS Certification.
  • An active driver's license, acceptable driving record and access to reliable transportation.
