Overview

We are looking for a compassionate and organized Social Worker to join our interdisciplinary team at our Adult Day Health Care Center in Sacramento. The Social Worker plays a vital role in supporting the emotional, psychosocial, and practical needs of our adult and senior participants. This position is ideal for someone who thrives in a collaborative, community-based care setting and is passionate about helping individuals maintain independence and quality of life.

Primary Responsibilities

  • Conduct initial and periodic psychosocial assessments for program participants.
  • Assist participants and their families in identifying and accessing community resources, benefits, and support services.
  • Provide counseling, crisis intervention, and emotional support as needed.
  • Develop and implement individualized care plans in coordination with the interdisciplinary team.
  • Facilitate family meetings and care conferences to address participant needs and goals.
  • Advocate on behalf of participants to ensure access to appropriate services and support.
  • Document all services and interventions in accordance with regulatory and organizational standards.
  • Lead or co-lead psychoeducational and support groups for participants and/or caregivers.
  • Assist with discharge planning and transitions of care when necessary.

Qualifications

  • Bachelor's or Master's degree in Social Work (BSW/MSW) from an accredited program.
  • Licensed or license-eligible in the state of California.
  • Experience in adult day health care, geriatric services, community health, or medical social work preferred.
  • Knowledge of community resources, Medi-Cal, IHSS, and other relevant programs.
  • Strong interpersonal, communication, and problem-solving skills.
  • Ability to work effectively as part of a multidisciplinary team.

Job Types: Full-time, Part-time

Pay: $ $35.00 per hour
