Position Overview:

In this role, you will supervise services provided to individuals receiving Day Program, Community Services, and In-Home Services. Your leadership will ensure quality, person-centered care and compliance with all service plans, documentation, and regulatory standards.

Requirements:

• Four year degree in a human service field and minimum of two years post-graduate experience with the I/DD population REQUIRED.
• Minimum of 2 years' experience working with individuals with I/DD
• At least 1 year of supervisory experience
• Drivers License with 3 Years Driving Experience
• Must have state minimum auto insurance.
• Must be able to provide at least 2 professional references to verify relevant work experience.
• Background checks will be performed on all viable applicants.
• Strong organizational, documentation, writing, and supervisory skills.
• Strong communication, organization, and leadership skills

About Our Company (A Caring Heart Case Management, Inc):

• Our company provides direct care services to individuals with intellectual and/or developmental disabilities.
• We provide a variety of different services including day programs, group homes, community services, and more
• Our direct care staff provides habilitative services aimed at providing care and guidance to our clients to help them learn, retain, and improve their skills

About this Position (Qualified Professional):

• We proudly service a portion of our clients within our 8 group homes managed 24/7 by our Habilitation Technician staff.
• Each participant is served by a team of individuals comprised of habilitation technicians and a supervisor, with a clinical designation of Qualified Professional.
• We offer on-going training to ensure that our QPs are knowledgeable in the most current therapeutic approaches and resources for the habilitation of individuals with intellectual/developmental disabilities. As a result, the individuals and families we support receive the services that best meet their needs.
• Our QPs assist the family with finding the best support staff for their loved one. The family and the individual served choose the staff person(s) to provide direct support services to them. QPs stay closely involved to ensure quality services are being provided.
• Our QPs supervise all services provided to the consumer through monthly observations at home, work, and/or in the community.
• Our QPs assist with developing the Individual Support Plan for accessing and using resources that can meet identified needs based on the individual's and family's strengths.
• Our QPs develop short range goals that the individual/family wants to work on in order to increase the individual's independence.
• Our QPs work with the care coordinator and other team professionals to ensure family needs are being met.

• Are you able to physically push, pull, or lift 25 pounds, unassisted, on a regular basis and up to 100 pounds unassisted occasionally?
• What populations have you worked with in your previous QP or direct support roles?
• How confident are you in writing Individual Support Plans (ISPs)?
• Will you be able to provide services on call as needed for emergencies as they arise?
• Are you currently credentialed as a Qualified IDD Professional (QP) under North Carolina regulations?
