Case Coordinator

Job Summary:
CASES is on the lookout for a Case Coordinator to join our Supervised Release Program. The Case Coordinator involves initiating Supervision Introduction/Orientation sessions with new entrants to outline necessary supervision conditions. The Case Coordinator will support participants in adhering to supervision schedules and legal proceedings, conduct regular assessments to identify service needs, and create collaborative service plans. The Case Coordinator emphasizes providing strength-based, gender-specific, and culturally sensitive services, guided by the principles of risk, need, and responsivity.

The aim is to reduce recidivism, foster community treatment engagement, and assist participants in achieving their personal goals for recovery and integration. The Case Coordinator should excel in teamwork, collaborate closely with a Peer Specialist, and be prepared for court duties and community fieldwork within NYC. Strong organizational and communication skills, along with efficient task prioritization, are essential. The initiative seeks to pioneer innovative methods to aid those in the Supervised Release Program facing significant challenges, ensuring their functional and programmatic success.

Salary:
The salary range for the Case Coordinator role is $57,680 a year. *Night/Weekend Differentials an additional $2,500 per year.

Shift Hours:
Varies from Monday - Friday from 9am to 5pm and/or 1pm to 9pm or Tuesday - Saturday from 9am to 5pm and/or 10am to 6pm.

Location Address:
Various locations throughout NYC inclusive of community field work.

What You Will Be Doing:

  • Provide supervision and community-based services to a caseload of pretrial participants.
  • Conduct Supervision introduction/orientation meetings with new participants to review supervision conditions within 24-48 hours of release.
  • Coordinate reporting schedule and the mandatory methods of communication required under the participant's supervision plan.
  • Work collaboratively with the court and other pretrial staff team to facilitate and ensure participants' successful completion of mandated supervision appointments and court appearances during the pendency of their legal case.
  • Review legal and court screening documentation and work with participants to analyze their offending behavior patterns and identify behavioral changes. Implement support that may reduce likelihood of any pretrial arrests.
  • Conduct service need assessments. Examine psychosocial needs and problems, including the severity of mental and substance use disorders that may affect participation, engagement, and the individual motivation in continuing in supervision and treatment plan.
  • Link participants to treatment based on their eligibility, preferences. Provide escort to community services as needed.
  • Maintain collaborative relationships with community providers (supported housing providers, Health Homes, care management programs, addiction treatment and mental health providers, peer-run providers, education, vocational training and employment services providers, legal services providers, medical care providers, and family providers).

What We Are Looking For:

  • Bachelor's degree or equivalent experience.
  • Minimum of three years' experience working in human services with individuals with substance use disorders, co-occurring mental health and substance use disorders and involvement in the criminal justice system.
  • Extensive knowledge of community treatment, support services and resources.
  • Problem solving techniques and motivational interviewing and case management approach skills to help motivate and engage individuals in any needed process of change.
  • Knowledge of the criminal justice system, court processes and alternative to detention/incarceration services
  • Must be able to sit for extended periods.

Additional, preferred skills we are looking for but are not required:

  • Bilingual in Spanish/English is a plus.
  • Experience using databases such as Salesforce.
