SBHC Lead Coordinator

The SBHC Lead Coordinator is responsible for coordinating and managing the daily operations of the School Based Health Center (SBHC). This role ensures the smooth functioning of the center, supports staff, and provides excellent service to students and families.
Includes all duties, responsibilities, competencies and abilities of SBHC Lead Coordinator with additional leadership responsibilities in specialized areas. The Lead Coordinator will assist the SBHC Leadership team to develop and maintain the Mobile Dental program.
Responsibilities include but are not limited to:
Coordinate daily operations of the SBHC.
Maintain confidentiality regarding patients, staff, and program matters.
- Assist in scheduling and maintaining clinical schedules.
- Locate and escort clients throughout the school buildings.
- Verifies insurance coverage as indicated by CS-HHC billing procedures.
- Receives telephone calls from patients and correctly and accurately records and transmits messages for Providers. Initiates outbound calls as needed.
- Coordinates patient care with colleagues, nursing, front desk outreach, case management staff and other agencies as necessary
- Prepares patients and rooms for exams according to specific procedures.
- Performs hearing and vision tests, Urine Dipsticks, Urine Pregnancy Tests and other CLIA waived tests when necessary.
- Maintains accurate logs of point of care testing, controls, and expiration dates.
- Ensures that the necessary equipment and information are available for exams.
- Participates in Outreach and Marketing development and activities to promote School Based Health and CS-HHC.
- Helps patients and their families understand how to utilize the resources of the CS-HHC.
- Serves as a backup site coordinator for the CT VFC Program and maintains all appropriate temperature logs.
- Performs all other duties as required
Requirements:
- High School Diploma/GED required.
- CPR certification required.
- Must have diploma from accredited Medical Assistant or Certified Nursing Assistant program.
- Bilingual English/Spanish strongly preferred.
- Five (5) years of experience as a Medical Assistant or Certified Nursing Assistant preferred. Previous medical office or school administration experience is preferred. Operations and/or grant management background preferred.
- Previous medical office experience as a medical assistant is preferred.
- Computer skills and data entry skills are required. Familiarity with Microsoft, Internet and E-mail is necessary. Computer skills are required with the ability to learn Epic, Centricity, EMR and Outlook.
- Ability to float to other departments or sites and perform all aspects of patient care.
- Excellent interpersonal skills, organizational skills, and written and verbal communications skills are essential.
- Ability to prepare patients for examination by performing preliminary physical tests; taking blood pressure, weight and temperature; verifies patient information by interviewing patient; recording medical history; confirming purpose of visit.
