Job Description

The In-Home Support Services (IHSS) Registered Nurse at Circle of Care Colorado plays a vital role in ensuring exceptional care for our pediatric and adult clients across the state. This position combines hands-on nursing expertise, caregiver supervision, and regulatory oversight to help individuals live safely and independently in their homes.  

Key Responsibilities 

  • Perform comprehensive assessments at admission, annually, and any time a client's condition changes. 
  • Develop and maintain individualized care plans in accordance with client needs and regulatory guidelines. 
  • Serve as the primary point of contact for clients, families, and caregivers regarding ongoing care needs or plan updates. 
  • Provide in-home supervision and consultation as agreed upon with clients or their authorized representatives. 
  • Educate clients, caregivers, and families on clinical needs, safety practices, and care expectations. 
  • Take the lead on care coordination with Medicaid case management agencies to ensure IHSS services are approved and CCC receives the authorized PAR in a timely manner. 
  • Provide consultative support when medical issues or questions arise during service delivery. 
  • Supervise caregivers, ensuring care delivery meets professional, ethical, and regulatory standards. 
  • Conduct skill validations, verify competencies, and document Attendant qualifications in alignment with IHSS and consumer safety procedures. 
  • Lead ongoing caregiver training initiatives to enhance safety, professionalism, and client-centered care. 
  • Ensure compliance with all state and federal standards. 
  • Review and audit care documentation and service records, and for accuracy and timeliness. 
  • Participate in agency quality improvement initiatives to enhance overall service outcomes. 
  • Identify potential risks within the home environment and implement proactive safety measures. 
  • Coordinate incident reporting, investigations, and follow-up documentation in accordance with CCC's policies and state regulations. 
  • Collaborate with the interdisciplinary team to ensure a consistent focus on safety and well-being
Qualifications
  • Active Registered Nurse (RN) license in the State of Colorado. 
  • Minimum of 2 years of experience in healthcare, home care, or community-based services; IHSS or HCBS experience is strongly preferred. 
  • Prior experience developing or facilitating caregiver education and competency assessments is strongly preferred. 
  • Demonstrated ability to resolve complex issues with professionalism and empathy. 
  • Strong organizational and documentation skills with meticulous attention to regulatory compliance. 

Physical and Travel Requirements 

  • Ability to lift up to 25 pounds as needed for office or in-home work. 
  • Comfortable traveling locally for home visits, caregiver supervision, and compliance reviews. 
  • Clear and effective verbal communication skills for engaging clients, caregivers, and regulatory partners.
