Join our dedicated healthcare team as a Certified Home Health Aide, where your compassionate care and attention to detail will make a meaningful difference in the lives of patients and their families. In this vital role, you will provide personalized assistance to individuals in their homes, supporting their health, safety, and well-being. Your expertise in patient observation, medication administration, and assisting with activities of daily living (ADLs) will ensure that clients receive high-quality, respectful care tailored to their unique needs. This position offers an opportunity to work closely with diverse populations, including seniors, individuals with disabilities, and those requiring specialized memory or hospice care.
Duties
- Deliver comprehensive personal care services such as bathing, grooming, dressing, and mobility assistance to promote independence and dignity.
- Monitor and record vital signs accurately while observing patient conditions for any changes or concerns.
- Assist with medication administration following prescribed instructions and maintain detailed medical documentation in compliance with HIPAA regulations.
- Support patients with activities of daily living (ADLs), including toileting, feeding, laundry, cleaning, and meal preparation to ensure a safe and comfortable environment.
- Utilize EMR (Electronic Medical Record) or EHR (Electronic Health Record) systems for documentation of patient care activities and medical records management.
- Operate medical equipment such as Hoyer lifts, catheters, and other assistive devices safely during patient transfers or care procedures.
- Provide specialized dementia and Alzheimer's care, memory care, hospice support, and working with individuals with developmental disabilities or disabilities requiring long-term care.
- Follow all safety protocols during procedures like CPR certification or catheter care to ensure patient safety at all times.
- Maintain a clean environment by performing light housekeeping tasks related to home & community care standards.
- Collaborate effectively with nursing staff and healthcare professionals to deliver coordinated patient care within home health settings.
Skills
- Strong ability to observe and assess patient conditions with keen attention to detail.
- Proven experience assisting in assisted living facilities, nursing homes, or home health environments.
- Proficiency in using EMR/EHR systems for accurate medical documentation and record keeping.
- Excellent skills in assisting with ADLs such as bathing, dressing, feeding, and mobility support.
- Knowledge of medical terminology relevant to home health caregiving practices.
- Ability to perform vital signs measurement accurately and respond appropriately to abnormal findings.
- Experience working with people who have dementia, Alzheimer's disease, developmental disabilities or require hospice or memory care services.
- Certification in CPR is required; additional certifications like medication administration or hospice training are advantageous.
- Familiarity with medical equipment including Hoyer lifts and catheter care devices for safe patient handling.
- Compassionate communication skills for working effectively with patients of all ages and backgrounds while respecting HIPAA privacy standards.
