Job Overview

We are seeking a compassionate and dedicated Designated Coordinator and Manager for our new Senior Adult Companion Care Department. This role is crucial in enhancing the quality of life for our lonely and isolated senior citizens, whether residing in assisted living facilities, nursing homes, or receiving care at home.

The position is responsible for the overall programmatic implementation of the person-centered approach. Aging Services for Communities provides services to senior citizens and people with intellectual and mental health disabilities in the community. This is a Part-Time hourly position.

This position will also include the duties of a Designated Coordinator who will provide supervision and support to direct support staff and is responsible for the evaluation of activities for individuals receiving services. The Designated Coordinator complies with the Support Plans for individuals receiving services according to federal and state regulations and the organization's policies and procedures to ensure the quality delivery of services to all individuals served.

JOB RESPONSIBILITIES:

The Designated Coordinator job responsibilities include:

  • 245D compliance and paperwork
  • Train, schedule, and provide ongoing support and evaluation of new Companion Care direct care staff
  • Manage services
  • Evaluate of the effectiveness of service delivery, methodologies, and progress on the person's outcomes
  • Communicate with individuals, guardians, case managers, behavior analysts, and other team members to decide on services needed to support each individual most effectively
  • Maintain and summarize data for reports
  • Coordinate appointments for individuals as requested
  • Problem solve when emergencies arise
  • Work direct care as needed
  • Assist and/or complete intakes with person-centered service plan with team members.
  • Developing and/or writing and implementation of each client's person-centered support plans, (CSSP) Coordinated Service and Support Plan, (SMA) Self-Management Assessment, and (IAPP) Individual Abuse Prevention Plan.
  • Maintaining all signatures as required by 245-D licensing rules and regulations.
  • Maintaining, updating all files as required by 245-D licensing rules and regulations.
  • Compliance with 245-D licensing rules and regulations.
  • Writing or quarterly and annual reports as needed or directed by the person served and/or team to fulfill person-centered plan.
  • Able to problem solve as needed and/or be creative to problem solve.
  • Coordination and ensuring the protection from abuse and general well-being of clients.
  • Staff orientation, training, and supervision regarded to person-centered needs and services as written in their specific plan.
  • Be available to receive calls during and outside of work hours from staff with questions or during emergencies.

APPLICANT REQUIREMENTS: To qualify for the position, applicants must:

  • Pass a Department of Human Services background check (Required)
  • Have a current Driver's License (Required)
  • Work independently and have strong organizational skills
  • Demonstrate strong written and oral communication skills
  • Proficiently use Microsoft Office
  • Have experience and understanding of 245D compliance

EXPERIENCE/EDUCATION: *Only resumes that meet the required education and experience will be reviewed

  • Must live in Minnesota and be willing to commute to the corporate office in Montgomery, MN.
  • 2 years' experience working with individuals who have high behavioral support needs, preferred
  • Minimum of 1 year of DC, Program Management/Supervisory experience, preferred

Qualified candidates must have one of the following:

  • Bachelor's Degree in a field related to human services, education, or health and one year of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older, or equivalent work experience providing care or education to vulnerable adults or children; or
  • an Associate Degree in a field related to human services, education, or health and two years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older, or equivalent work experience providing care or education to vulnerable adults or children; or
  • a Diploma in a field related to human services, education, or health from an accredited postsecondary institution and three years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older or equivalent work experience providing care or education to vulnerable adults or children; or
  • a Minimum of 50 hours of education and training related to human services and disabilities; and four years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older under the supervision of a staff person who meets the qualifications identified above.
