Our Residential Brain Injury programs provide support to our residents with brain injuries so they may lead lives as independently as possible.

Stephens Farm is hiring a QMAP Coordinator.
Pay rate: $48,000.00 to $50,000.00, overtime eligible.
General Statement of Duties:
Responsible for overseeing medication provision for residents of Adeo
Performs all duties and responsibilities of a Direct Care Provider (DCP) as needed
Models and coaches DCP staff in onsite resident engagement
Trains newly certified Qualified Medication Administration Personnel (QMAP); provides ongoing modeling, coaching and support as needed

Supervision Given: None
Supervision Received: Care Manager
Lead QMAP Expectations:
Train new QMAPs on shift responsibilities regarding med administration
Print updated medication lists for each resident and place in respective medical appointments books; file previous month's lists in resident charts
Review last month's Medication Administration Records (MAR) and issue corrections; MARs must be in the med room ready for use by the end of the day on the last working day of the month
Complete cycle-census each month when the pharmacy faxes it prior to cycle delivery
Put out cycle medications when they are delivered; verify that every medication for every client is included, fill out and fax to the pharmacy a cycle discrepancy form (found in the drawer in the med room) if necessary
Check stock on manual re-order medications and re-order as needed so as not to interrupt medication supply
Ensure Medication Reminder Boxes (MRB) are filled as needed
Ensure residents' weights are recorded monthly
Alliance orders and inventory
Assist with physical/behavioral health appointments as needed
Other duties as assigned
Resident Engagement Expectations:
Model for QMAPs thoughtful, intentional engagement during less structured or busy parts of regular shifts.
Work with Person Centered Engagement Manager on strategies to encourage and coach QMAPs to improve quality of engagement in everyday resident activities.
Qualifications:
At least one year of experience providing direct care preferred.
First Aid/CPR, QMAP and CPI certifications must be up-to-date.
Must pass MVR and hold a valid driver's license with 5 years driving experience.
EMR/EHR application experience preferred.
Excellent communication skills, including the ability to speak, read and write English.
Ability to speak Spanish is a plus.

Exert negligible amount of force constantly
Lift, carry, push, pull, stand, walk, sit, reach, and crouch for up to 30 minutes at a time
