Job Summary:

We are looking for someone who is empathetic, adaptable, resourceful, and confident in engaging with diverse populations in both community and field-based settings.

In this role, you will provide outreach, assessment, short-term case management, treatment planning, crisis support, and resource coordination to individuals experiencing homelessness, youth and families in crisis, or adults with recurring emergency room utilization due to unmet behavioral health needs. You will partner with community organizations, hospitals, and internal teams to promote safety, stabilization, and long-term wellness.

Key Responsibilities:
· Provide meaningful outreach and engagement to individuals experiencing homelessness, adults with frequent emergency room use, or youth and families in crisis, helping them access needed services and supports
· Build trusting relationships and deliver person-centered care to help individuals identify goals, access treatment, and connect with long-term supports
· Conduct intakes, assessments, and screenings to determine eligibility for services and link individuals to appropriate programs such as CPRC or community support services
· Deliver crisis intervention, stabilization services, and short-term case management as needed
· Offer resource navigation, including support with accessing housing programs, treatment providers, income supports, medications, and basic needs
· Maintain a caseload of program participants, addressing immediate needs and assisting with preparation for longer-term service enrollment
· Document services, outreach efforts, and clinical notes accurately and within required timeframes
· Collaborate with hospitals, community mental health providers, substance use treatment agencies, and partner organizations to ensure continuity of care
· Conduct outreach in community settings such as homeless camps, emergency rooms, homes, and other field-based environments
· Develop individualized treatment or recovery plans and monitor progress toward goals
· Participate in team meetings, supervision, and staff development activities
· Support Youth ERE or Adult ERE functions as applicable, including therapy for youth (if qualified), family sessions, or comprehensive behavioral health assessments
· Maintain electronic medical records and compile reports as required
· Engage in quality assurance processes and training to support program compliance and professional growth

Education, Experience, and/or Credential Qualifications:
· Bachelor's degree in Psychology, Sociology, Social Work, or another related human service field, including a Behavioral Health Support (BHS) degree from an accredited institution
· Advanced knowledge of mental health issues
· Must have a valid driver's license
· Must use personal vehicle for transportation of consumers/clients
· Must have auto liability insurance including uninsured motorist coverage

Additional Qualifications:
· Must be able to pass background checks including criminal record, driving record, abuse/neglect, and fingerprint screening
· Must maintain an acceptable driving record and current auto insurance
· Must be able to read and interpret procedures, safety rules, and technical instructions; write reports; and communicate effectively with residents, staff, families, and community partners
· Ability to perform basic math (addition, subtraction, multiplication, division)
· Ability to follow instructions, multitask, prioritize demands, and remain composed during crises
· Ability to establish effective relationships through phone and in-person interactions

Physical Requirements:
ADA Consideration – Sedentary work: Exerting up to 10 pounds of force occasionally and/or a negligible amount of force frequently to lift, carry, push, pull, or otherwise move objects, including the human body. Requires repetitive movements of hands, fingers, and arms for typing and/or writing. Primarily seated work with occasional walking or standing. Jobs are considered sedentary if walking and standing are required only occasionally and all other sedentary criteria are met.
