Job Summary:
The Transitional Care Medical Assistant participates in quality and service improvement initiatives to improve patient care.
Duties/Responsibilities:

Works in a team model to improve the patient's experience, create better communication and access to care, and support processes that will improve patient satisfaction.

Develops different approaches to minimize gaps in coverage; identifies and reaches out to patients with gaps in care; collaborates with staff for gap closure and to complete quality care goals; documents activities related to gap closure initiatives.

Pre-screens all scheduled patients via Electronic Medical Records (EMR) to identify opportunities to close gaps.

Follows outreach protocols to schedule patients for necessary visits, labs, procedures, etc.

Assists in identifying and monitoring patients with complex disease states and provides patient/family education and direction within the MA scope.

Assists in the coordination of medical services, transitions of care, and makes appropriate referrals for community services for the patient and family/caregivers.

Assists clinical teams with proper documentation/requirements for quality requirements.

Organizes and disperses referral authorization forms to clinics as needed and within regulatory requirements.

Retrieves reports from payor portals for patient outreach initiatives.

Participates in chart scrubbing, retrieval of medical records and data collection to support monthly initiatives for quality improvement and to close outstanding care gaps for NOAH patients.

Update health maintenance as indicated during chart auditing, schedule appointments as necessary, order testing per protocols, obtain medical records and test results for gap closure, review Epic reports.

Ability to use multiple insurance and quality databases to support successful care gap closure for patients.

Responsible for ongoing data submission to health plans and as assigned.

Attends, participates, and/or assists in meetings, trainings, community outreach activities, continuing education opportunities, and other activities as required.

Performs other related duties as assigned.

Ability to speak English and Spanish preferred.
Education and Experience:
Required:
-
High School Diploma or GED.
-
Certified or Registered Medical Assistant OR completion of a Medical Assistant program that meets the BOMEX training guidelines OR completion of a United States Armed Forces medical services training program OR meet necessary guidelines as described in Arizona Board of Medical Examiners Administrative Code, Title 4, Chapter 16, Article 4, Section R , paragraphs A and B prior to start date OR have 1 year of Medical Assistant Experience.

Basic Life Support (BLS) certification.

1+ years of outpatient, ambulatory care, specialty care or community health experience.
Preferred:
-
Associate's degree in a related field or equivalent work experience.

2+ years of experience in outpatient clinical settings working with PCPs.

2+ years of Medical Assisting experience.
Other Requirements:

New Hires are required to pass pre-employment background check and drug testing (effective 11/1/2022).

Current Level 1 Fingerprint Clearance Card or must pass fingerprint clearance within 7 days following hire.
