Job Summary

Ace Physician is seeking a dedicated and compassionate Nurse Practitioner (NP) to provide in-home medical care for adult and geriatric patients. This role is ideal for a clinician who values patient-centered care and enjoys delivering healthcare in a more personal, comfortable setting — the patient's home.

Responsibilities

  • Conduct comprehensive home visits and physical examinations
  • Manage chronic diseases and coordinate care plans with physicians
  • Order, review, and interpret diagnostic tests
  • Accurately document encounters in the EMR system
  • Educate patients and families on medication management and preventive care
  • Ensure compliance with HIPAA and clinical standards
  • Maintain clear communication with the care coordination team

Qualifications

  • Licensed Nurse Practitioner (NP) in the State of Texas
  • DEA license (or eligibility to obtain)
  • Minimum 1 year of NP experience in primary care, home health, or geriatrics (preferred)
  • Excellent communication and clinical decision-making skills
  • Reliable transportation and valid driver's license
