TriHealth is actively recruiting for a PRN Speech Pathologist for PACE of Cincinnati. PACE Center is located in Norwood Ohio. Program of All-Inclusive Care for the Elderly (PACE) is a Medicare and Medicaid program that helps people meet their health care needs in the community instead of going to a nursing home or other care facility.
The PACE Speech-Language Pathologist is responsible for the speech therapy program for PACE participants. The PACE Speech-Language Pathologist assesses, diagnoses, and treats various communication disorders including speech articulation, swallowing difficulties, language comprehension, fluency, and voice quality. The PACE Speech-Language Pathologist cares for participants in the PACE Center, Skilled Nursing Facilities, Assisted Living Facilities, and community living settings as indicated. The PACE Speech-Language Pathologist is a member of the Interdisciplinary Team (IDT) and is responsible for completing speech therapy assessments for new PACE participants and speech therapy reassessments on enrolled PACE participants as part of the PACE IDT care planning process. The PACE Speech-Language Pathologist provides speech therapy education to participants, caregivers, and PACE staff.
Job Requirements:
Master's Degree in Speech-Language Pathology. (Required)
1 - 2 years of experience working with a frail or elderly population, or in the absence of such knowledge willingness to have training from PACE of Cincinnati on working with a frail or elderly population upon hiring. (Required)
Other Licensed by Ohio in Speech-Language Pathology and Audiology. Upon Hire Required
Basic Life Support (BLS) Certification. Preferred
Job Responsibilities:
Participates as a PACE Interdisciplinary Team (IDT) member and is responsible for completing speech therapy assessments for new PACE participants and speech therapy reassessments on enrolled PACE participants as part of the PACE IDT care planning process.
In collaboration with the primary care provider and IDT, develops and implements speech therapy care plans for participants, identifying and considering the medical, physical, social, and emotional needs of PACE participants. Attends IDT meetings and takes part in the participant care planning process.
Coordinates and implements 24-hour care delivery that meets PACE participant needs across all care settings.
Working collaboratively with the PACE primary care provider, treats PACE participants for communication disorders, including speech articulation, swallowing difficulties, language comprehension, fluency, and voice quality.
Monitors PACE participants for changes in their health or psychosocial status requiring a speech therapy reassessment. Promptly communicates pertinent changes in participant health status to the IDT.
Completes and maintains speech therapy documentation in the PACE EHR in accordance with the federal PACE regulation and organizational policy.
Serves as a speech therapy subject matter expert and communicates best practices for a frail elderly population to the IDT, participants, and caregivers. Supports the PACE QI program and participates in PACE QI activities as assigned.
