Location: Delhi, NY 13753
Duration: 13 Weeks
Shift: Days | 3×12-hour shifts
Local Pay Range: $58–$63/hour
"Navitas Healthcare, LLC" is seeking a skilled and patient-focused Physical Therapist to provide high-quality rehabilitation services in a primarily outpatient setting, with occasional inpatient coverage in acute care and rehabilitation units. The PT will evaluate patients, design individualized treatment plans, and deliver evidence-based interventions across orthopedic, neurological, chronic, and sports-related conditions. This role supports a diverse patient population while ensuring safe, effective, and patient-centered care.
Responsibilities- Perform comprehensive patient evaluations, including new patient assessments (60-minute appointments).
- Develop and implement individualized treatment plans based on clinical findings.
- Deliver outpatient PT services for orthopedic, neurological, chronic pain, and sports injuries.
- Provide occasional inpatient coverage in acute care and swing bed rehab units.
- Monitor patient progress, adjust treatment plans, and document care using Epic EMR.
- Utilize therapeutic modalities such as ultrasound, e-stim, mechanical traction, paraffin, treadmills, UBE, and other exercise equipment.
- Educate patients and caregivers on exercises, injury prevention, and home programs.
- Collaborate with physicians, nursing staff, and interdisciplinary rehab teams to coordinate care.
- Maintain a full outpatient caseload of 15 patient slots per 8-hour day (30-minute sessions).
- Adhere to all regulatory, safety, and quality standards.
- Provide occasional Saturday coverage as needed.
Required:
- Active Physical Therapy license in the state of New York
- Current BLS certification
Preferred:
- Outpatient physical therapy experience
- Ability to manage a high-volume outpatient schedule
- Strong clinical evaluation and treatment planning skills
- Experience with orthopedic, neurological, chronic condition, and sports-related rehab
- Comfort with occasional inpatient/acute care coverage
- Excellent communication, documentation, and patient education abilities
