Duties

  • Assess patients' physical conditions through detailed evaluations, utilizing your strong understanding of physiology and anatomy to develop tailored care plans.
  • Implement evidence-based physiotherapy techniques such as trigger point therapy, orthopedics interventions, and geriatrics-focused exercises to promote healing and functional improvement.
  • Coordinate discharge planning and collaborate with multidisciplinary teams to ensure seamless patient transitions from hospital to home or community settings.
  • Manage care across various environments including inpatient hospital units, outpatient clinics, home health visits, and pediatric or geriatric populations.
  • Document patient progress accurately using electronic medical record (EMR) systems, ensuring compliance with healthcare regulations and standards.
  • Educate patients and their families on physiotherapy exercises, injury prevention strategies, and self-care techniques to foster long-term wellness.
  • Participate in ongoing professional development to stay current with advancements in physiotherapy practices and medical terminology.

Requirements

  • Valid licensure as a Doctor of Physical Therapy (DPT) or equivalent credential in the relevant jurisdiction.
  • Proven experience working in inpatient hospital settings, outpatient clinics, or home health environments with a focus on patient care.
  • Strong knowledge of physiology, anatomy, and medical terminology essential for accurate assessments and treatment planning.
  • Hands-on experience with discharge planning, care plans development, and care coordination across multiple healthcare settings.
  • Familiarity with EMR systems for documentation and patient record management.
  • Experience working with diverse populations including pediatrics, geriatrics, orthopedics, and those requiring acute care interventions.
  • Ability to perform trigger point therapy and other physiotherapy techniques effectively.
  • Excellent communication skills to collaborate with healthcare teams and educate patients effectively.
  • Home & community care experience is highly desirable for providing comprehensive support beyond clinical settings.
