Position Summary:

Hometown Healthcare is a private primary care practice that is Veteran owned and operated and is located in Prescott Valley Town Center. We service a wide range of patients in our community from newborn to geriatric along with servicing multiple facilities in town by providing mobile primary care to residents. As a nurse practitioner with Hometown Healthcare, you will be expected to conduct physical examines, interpret lab test and monitor patients' overall health. Team spirit and a positive attitude are a must within the culture of Hometown Healthcare, as you will be working closely with other healthcare professionals.

Responsibilities

  • Examine patients and review/update their medical records
  • Order and study diagnostic tests (blood tests, X-rays etc.)
  • Diagnose health conditions and illnesses
  • Propose treatments for chronic and infectious diseases
  • Prescribe medications in accordance to best practices
  • Maintain accurate records and schedules
  • Inform patients about their health conditions and prescribed medications
  • Educate patients and their family members on diseases, epidemics and prevention
  • Promote good health practices including preventative medicine
  • Advance professional knowledge with continuing education

  • Expertise in primary care, acute care and chronic care practices
