Bowes In Home Care is a home health organization dedicated to delivering quality, patient-centered care. We are seeking an Intake LPN Coordinator to join our team and support the referral, intake, and scheduling process to ensure timely and appropriate initiation of home care services.

Position Summary

The Intake LPN Coordinator is responsible for reviewing new referrals, verifying patient insurance coverage, and coordinating with patients, families, and field staff to schedule home care services. This role requires a detail-oriented Licensed Practical Nurse (LPN) with strong communication skills and a background in home health or clinical coordination.

Key Responsibilities

  • Review all new referrals for clinical appropriateness and completeness of documentation
  • Verify insurance coverage and eligibility, securing authorizations as needed
  • Coordinate care between patients, families, and field staff to ensure timely scheduling of services
  • Maintain accurate and complete intake records in compliance with agency, state, and federal guidelines
  • Ensure adherence to Medicare, Medicaid, and private insurance requirements
  • Communicate professionally and compassionately with patients, families, and staff
  • Support agency leadership in maintaining efficient intake processes and high-quality standards of care

Qualifications

  • Current Illinois LPN license (required)
  • Prior experience in home health, case management, or intake coordination (preferred)
  • Strong knowledge of insurance verification, authorizations, and clinical documentation
  • Excellent organizational, communication, and problem-solving skills
  • Ability to manage multiple priorities in a fast-paced environment
