About the Role

We're hiring a Director of Quality Assurance (QA Director) to lead and elevate our Quality Assurance program during an exciting period of expansion. We are an extremely rapidly growing company with significant growth opportunities, and this role will be central to strengthening documentation quality, compliance, and performance improvement as we scale.

You'll provide strategic direction and hands-on oversight of agency-wide QA practices, ensuring high-quality clinical documentation, regulatory compliance, CHAP survey readiness, and strong execution of QAPI (Quality Assurance & Performance Improvement) initiatives. You will also play a key role in managing documentation support related to ADRs (Additional Documentation Requests).

Key Responsibilities

  • Provide leadership, oversight, and strategic direction for the Quality Assurance department.
  • Conduct and oversee QA reviews of all visit types and clinical notes, ensuring accuracy, completeness, and adherence to standards.
  • Deliver education, coaching, and training to clinicians and QA staff on documentation expectations and best practices.
  • Ensure agency-wide compliance with internal policies, payer requirements, and state/federal regulatory documentation standards.
  • Lead QAPI initiatives by tracking performance measures, identifying trends, implementing corrective actions, and driving continuous improvement.
  • Maintain CHAP accreditation readiness, including mock surveys/audits, survey coordination support, and corrective action planning.
  • Manage and support ADR responses, ensuring timely, accurate, and complete documentation submission while improving processes to reduce future risk.
  • Partner with clinical leadership and operations to improve documentation workflows and strengthen quality outcomes.
  • Create and maintain QA reporting, feedback loops, and follow-up processes to ensure sustainable improvement.

Required Qualifications

  • 5+ years of Quality Assurance management experience in a healthcare clinical environment (home health strongly preferred).
  • Strong knowledge of clinical documentation standards, compliance expectations, and quality review processes.
  • Direct experience with CHAP surveys/accreditation (preparation, participation, and corrective actions).
  • Direct experience handling ADRs (coordination, documentation review, and submission support).
  • Experience leading or supporting QAPI / performance improvement initiatives.
  • Strong communication skills and demonstrated ability to train, coach, and influence clinicians and staff.

Preferred Qualifications

  • Familiarity with Medicare home health Conditions of Participation (CoPs) and payer documentation requirements.
  • Experience building QA programs, audit tools, training materials, and scalable processes.

Compensation

  • Salary is subject to experience and open for negotiation
