Job Summary:

Guggenheimer Health and Rehabilitation is seeking a compassionate and reliable Transportation personnel to join our dedicated team. The Transportation personnel will be responsible for safely transporting residents to and from medical appointments, activities, and other essential appointments, scheduling appointments and other transportation. This role requires a detail-oriented individual who can ensure the comfort and safety of our residents during transportation and is committed to providing excellent care.

Key Responsibilities:

Transportation and Safety:

  • Transport residents to and from scheduled appointments, activities, and other designated locations in a safe and timely manner.
  • Assist residents in and out of vehicles, ensuring secure wheelchair positioning and seat belt use.
  • Monitor and follow all safety protocols during transport.
  • Schedule appointments and other transportation

Resident Care and Support:

  • Ensure residents are comfortable during transit and attend to any needs that arise.
  • Maintain open communication with nursing and support staff regarding resident transport needs and any changes in schedule.
  • Provide respectful, compassionate interaction with all residents, fostering a welcoming and supportive environment.

Vehicle Maintenance:

  • Perform basic checks on the facility vehicle(s), including fuel, oil, and tire levels, to ensure readiness for trips.
  • Report any maintenance needs or vehicle concerns to the supervisor promptly.

Documentation and Communication:

  • Accurately document transport logs, resident information, and trip details.
  • Communicate any resident concerns or notable occurrences during transport to appropriate healthcare staff.

Qualifications:

  • High school diploma or equivalent.
  • Valid driver's license with a clean driving record.
  • Experience working with elderly or disabled individuals, preferably in a healthcare or rehabilitation setting.
  • CPR and First Aid certification (or willingness to obtain).
  • Ability to lift, move, and assist residents as needed.
  • CNA License
