Wedgewood Healthcare and Rehabilitation is seeking a compassionate and skilled Director of Social Services to lead our department in delivering exceptional, resident-centered care. This is an opportunity for a strong leader with clinical insight, regulatory knowledge, and a genuine passion for enhancing the lives of residents.

Key Responsibilities
  • Develop, implement, and maintain departmental policies and procedures in compliance with all regulatory requirements.
  • Recruit, mentor, and manage a dedicated team of social services professionals.
  • Uphold and advocate for the Resident Bill of Rights, fostering dignity and respect in every interaction.
  • Actively participate in interdisciplinary care planning meetings.
  • Lead discharge planning to ensure smooth, effective resident transitions.
  • Monitor and evaluate the quality of social services to continuously improve resident outcomes.
  • Collaborate with other departments to maintain compliance with documentation standards.
  • Prepare and present Quality Assurance (QA) reports as needed.
  • Serve as the facility's Grievance Officer, addressing resident and family concerns with empathy, fairness, and professionalism.
Qualifications
  • Bachelor's Degree in Social Work (BSW) or related human services field (required)
  • Minimum of 2 years of experience in a healthcare or medical setting
  • Strong knowledge of federal and state regulations related to long-term care social services
