Company Overview

'Wayne Provision operates under a continuum contract to provide Level 2 and Level 3 services in partnership with the Tennessee Department of Children's Services across the state to serve and care for youth in Tennessee foster care.

Wayne Provision works to provide a temporary, caring, supported home environment for eligible youth until reunification or a plan for permanency is achieved.

We envision a Tennessee where HOPE is restored through offering safe, loving, and skilled foster families to meet the needs and provide for the overall wellbeing of children in care.

General Information:

The Placement Specialist will respond to all DCS referrals and continuum referrals within 24 business hours. The Placement Specialist will be on call for any after-hour referrals or temporary placement needs.

Duties/Responsibilities:

  • Ensure all necessary documents are obtained when accepting youth into the Wayne Provision Foster Care Program including CS-0727: Initial Intake, Placement and Well-Being Information and History, Authorization for Routine Health, Informed Consents for any psychotropic medications-if necessary, Education Passport/School Enrollment Letter, birth certificate, social security card, TennCare card, and custody order.
  • Participate in weekly calls with foster care and foster home staff to ensure appropriate matches into foster homes.
  • Abide by DCS policy 16.46: Child/Youth Referral and Placement as it relates specifically to the CANs assessment, Placement Exception requirements, and Safety Plans.
  • Participate in CFTMs for all youth in the Wayne Provision Program.
  • Report all moves to the appropriate staff within 24 business hours. Review TFACTS for correct entry.
  • Track and review all (internal) 30-day notices submitted by Wayne Provision Foster Parents.
  • Participate in agency meetings that discuss the youth placed in Wayne Provision Foster Homes: Perm Meeting, Foster Home Team/Case management staff meeting, Safety Plan Review, Internal disruption meetings, etc.
  • Participate in site visits with youth in person or virtually in our facilities, monitoring for stepdown to a foster home placement.
  • Maintain spreadsheets for youth in placement and referrals received from DCS.
  • Coordinate with Foster Care Program Specialist on reviewing status of Safety Plans.
  • Support the Wayne Halfway House, Inc. mission statement and approach to diversity, equity, and inclusion.
  • Demonstrate cultural knowledge, awareness, and sensitivity.

Education and Experience:

  • Bachelor's Degree from an accredited institution with a major in social work or a related field preferred. If the degree is in criminal justice, there must be a juvenile justice component.
  • Master's Degree in the social sciences or volunteer, practicum or intern experiences in programs/facilities that work with children and families preferred but not required.
  • At least five years' experience in a juvenile facility is preferred but not required.
  • Must have a current valid license in the state of employment to transport youth.

Physical Requirements:

  • Prolonged periods of sitting, standing, and driving.
  • In-person interaction required.
  • Must be able to lift up to 15 pounds at times.
  • Must pass a drug test.
