Job Title: Home Health Aide

As a Certified Nursing Assistant (CNA) with Home Health Aide (HHA) certification at Partners CA Home Health, you will provide compassionate and reliable personal care services to patients in the comfort of their homes. You'll work under the supervision of a Registered Nurse or Therapist to assist patients with daily living activities, promote safety, and support their independence. Your role is essential in maintaining the highest standard of home-based care and enhancing each patient's quality of life.

What you'll do

  • Assist patients with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, and mobility.
  • Perform basic healthcare tasks under RN supervision, such as checking vital signs, monitoring intake/output, and reporting changes in condition.
  • Provide personal hygiene care, including skin care, oral care, and hair care, while maintaining patient dignity.
  • Support patients with ambulation, transfers, and prescribed exercises.
  • Assist with light housekeeping, meal preparation, and maintaining a safe home environment.
  • Observe, document, and report patient progress or changes to the supervising nurse promptly.
  • Provide emotional support and companionship to patients and families.
  • Follow infection control procedures and maintain HIPAA confidentiality at all times.
  • Assist with simple prescribed therapy activities under professional supervision.

Basic qualifications

  • Current Certified Nursing Assistant (CNA) certification in the State of California.
  • Current Home Health Aide (HHA) certification recognized by the California Department of Public Health.
  • Current CPR/BLS certification.
  • Reliable transportation and valid California driver's license.
  • Compassionate attitude and commitment to high-quality patient care.

Preferred qualifications

  • Prior experience in home health care or long-term care settings.
  • Familiarity with electronic documentation systems (Alora Plus preferred).
  • Knowledge of infection control and universal precautions.
  • Bilingual (English/Spanish) preferred.
