Position Summary

The Telehealth Counselor will provide remote supportive counseling, care coordination, and behavioral health guidance to older adults enrolled in the Connecticut Home Care Program for Elders (CHCPE). This role supports clients in maintaining independence, emotional stability, and quality of life within their homes. The Counselor will meet with clients virtually, provide ongoing assessments, document progress, and collaborate closely with case managers and CHCPE program staff.

This position requires a Master's degree in Counseling, Social Work, Psychology, Marriage & Family Therapy, or a related behavioral health field. Experience with older adults, chronic conditions, dementia, or long-term care programs is highly preferred.

Key ResponsibilitiesTelehealth Counseling & Support

  • Provide virtual counseling sessions to CHCPE clients focusing on emotional well-being, coping strategies, adjustment to aging, and chronic condition support.
  • Conduct psychosocial assessments to identify needs, risk factors, and support areas.
  • Deliver short-term, solution-focused therapy and behavioral interventions suitable for elderly populations.
  • Assist clients with stress management, grief counseling, social isolation, and caregiver-related concerns.

Care Coordination & CHCPE Collaboration

  • Work closely with CHCPE case managers to support individualized care plans.
  • Participate in interdisciplinary meetings as needed to discuss client status and service needs.
  • Coordinate with caregivers, family members, or natural supports (with permission) to reinforce goals and improve outcomes.
  • Recommend referrals to community and medical services when appropriate.

Documentation & Compliance

  • Maintain accurate, timely electronic records in compliance with CHCPE program standards and HIPAA.
  • Complete assessments, session notes, and progress reports according to agency timelines.
  • Follow telehealth best practices, confidentiality requirements, and informed consent procedures.
  • Ensure that services delivered align with CHCPE program rules and agency policy.

Minimum Qualifications

  • Master's degree (required) in:
  • Social Work (MSW)
  • Counseling (MA/MS/MEd)
  • Psychology (MA/MS)
  • Marriage & Family Therapy (MFT)
  • Mental Health Counseling
  • Licensed or license-eligible in Connecticut preferred (LPC, LMFT, LCSW, LMSW acceptable).
  • Experience with older adults, home care, Medicaid programs, or long-term care preferred.
  • Familiarity with telehealth platforms and virtual engagement strategies.
  • Strong interpersonal, communication, and clinical documentation skills.
  • Ability to work independently while meeting agency and CHCPE requirements.

  • Remote/telehealth position with flexible scheduling.
  • Requires reliable internet access and a confidential work environment.
  • Must attend virtual team meetings, supervision, and required training.

Compensation

  • Competitive hourly or per-session rate based on experience and licensure.

Pay: $40.00 per hour

Expected hours: 10.0 per week
