Job Summary

We are seeking a compassionate and dedicated Masters Level Social Worker / 'Mental Health Therapist to join our dynamic rest home team. In this vital role, you will provide comprehensive mental health services to diverse patient populations, fostering recovery, resilience, and well-being. Your expertise will help individuals navigate emotional challenges, behavioral issues, and mental health conditions through evidence-based therapies and personalized care plans. Duties

  • Conduct thorough patient assessments to understand individual mental health needs, including diagnostic evaluations and intake interviews.
  • Develop and implement tailored treatment plans utilizing various therapeutic modalities such as behavioral therapy, psychodynamic therapy, Christian counseling, and group therapy.
  • Deliver individual and group counseling sessions focused on mental health counseling, social work principles, aging issues counseling, geriatrics, and other specialized areas.
  • Apply applied behavior analysis (ABA) techniques to support patients with behavioral health challenges.
  • Manage case documentation accurately within electronic medical records systems while adhering to HIPAA regulations to ensure patient privacy and confidentiality.
  • Perform ongoing patient care activities including progress monitoring, adjusting treatment strategies as needed, and providing case management support.
  • Collaborate with multidisciplinary teams—including social workers, medical professionals, and community care providers—to coordinate comprehensive patient care.
  • Conduct social work assessments and facilitate home & community care initiatives to promote holistic recovery outside clinical settings.
  • Maintain compliance with medical terminology standards and ensure all documentation aligns with healthcare regulations.
  • Engage in continuous professional development by staying current with best practices in mental health counseling and behavioral health interventions.

Requirements

  • Master's degree or higher in Psychology, Social Work, Counseling, or a related field from an accredited institution.
  • Valid licensure or certification as a Mental Health Therapist or equivalent in the respective state or region.
  • Proven experience in clinical counseling, including individual and group therapy settings.
  • Strong knowledge of behavioral therapy techniques, applied behavior analysis (ABA), psychodynamic approaches, and Christian counseling principles is highly desirable.
  • Familiarity with diagnostic evaluation processes for mental health conditions and aging issues counseling.
  • Experience working with diverse populations including geriatrics and patients facing complex emotional or behavioral challenges.
  • Ability to handle sensitive information with discretion while maintaining HIPAA compliance.
  • Excellent communication skills for effective patient assessment, treatment planning, and interdisciplinary collaboration.
  • Proficiency in medical documentation systems and understanding of medical terminology related to mental health care.
  • Home & community care experience is a plus for supporting patients outside traditional clinical environments.
