We are in compliance with all applicable federal, state, and local ordinances and regulations. We offer a variety of home health care services, which can assist you and your loved ones in their home. TLC provides intermittent, part-time, or extended hours of all our services.
The Director of Nursing (DON) is a key leadership role responsible for overseeing all clinical operations within the home health care agency. The DON ensures high-quality patient care, regulatory compliance, and efficient coordination between clinicians, caregivers, clients, and administrative staff. This role provides strategic direction, supports staff development, and upholds the agency's mission to deliver compassionate, safe, and effective care in the home setting.
Responsibilities:
Clinical Leadership & Oversight
- Provide direct supervision, mentorship, and support to RNs, LPNs, HHAs, and other clinical staff.
Regulatory Compliance
- Ensure ongoing compliance with state and federal regulations (CMS, DHS, OSHA, HIPAA).
- Maintain accreditation standards, conduct internal audits, and coordinate external surveys and inspections.
- Develop, interpret, and implement nursing policies and procedures.
Quality Improvement
- Lead performance improvement initiatives to enhance patient outcomes and operational efficiency.
- Monitor key clinical indicators and implement corrective action plans where needed.
- Participate in root-cause analyses and develop strategies to prevent adverse events.
Care Coordination and Communication
- Act as the primary clinical liaison between physicians, caregivers, families, and external providers.
- Resolve escalated clinical concerns and provide guidance on complex patient situations.
- Ensure timely communication and documentation across interdisciplinary teams.
Operational & Administrative Duties
- Manage clinical staffing schedules to ensure adequate coverage for all client needs.
- Participate in budgeting, cost control, and resource planning.
- Review and approve clinical documentation, including OASIS, assessments, and plans of care.
- Ensure all patient care delivered meets professional standards, clinical guidelines, and agency protocols.
- Conduct routine clinical case reviews and oversee care plans to ensure accuracy, timeliness, and quality.
