Overview:
The RN Hospice Case Manager (Pediatrics) provides outstanding patient care based upon the nursing process; assessment, nursing diagnosis, planning, implementation and evaluation, and Admissions to the program.
Duties and Responsibilities:
Th RN Peds Case Managers work collaboratively with the pediatric Physicians, Patient Care Coordinator, Social Worker, Chaplain and Certified Nursing Assistants as an integral member of the interdisciplinary team.
The Case Managers assume the overall responsibility for organizing patient care, develop the interdisciplinary plan of care, provide comprehensive nursing assessments, teach appropriate palliative interventions to family members, assess and implement a pain/symptom management program to enhance quality of life and provide the highest level of hospice care to our patients or palliative care to our pediatric patients.
Education and Experience:
- Graduate of a School of Nursing ADN or BSN
- Hospice and/or home health experience preferred, not required
- Pediatric experience preferred but willing to train the correct person
- Must be able to work in the field independently and in a team approach
- Must be able to manage a case load of 12-16 patients
- must have excellent communication skills, organizational skills and understand the basics of home care, or hospice or pediatric palliative care (willing to training)
- Must be able to navigate a EMR system
Required Licensure and Certification:
- Current license to practice as a Registered Nurse in the State of Nevada. (Unrestricted)
- RNs must also be a licensed driver and possess an automobile that is insured in accordance with state and/or organizational requirements and is in good working order. Must maintain a clean driving record.
This is a full-time position. 5 days per week, 8 - hour shifts.
