Responsibilities:
- Deliver comprehensive primary care at home/senior communities visits to patients
- Represent Bloom Healthcare in the Dallas / Forth Worth Market as a Bloom ambassador supporting our new market growth including market analysis to understand the local healthcare landscape and competitors.
- Strategic planning to grow your patient panel and deliver comprehensive primary care at home visits to 10 patients per day.
- Build trusted relationships by engaging with key members of our patients' care teams, including assisted living facilities, hospitals, clinics, and community organizations, to facilitate coordination of care.
- Collaborate with interdisciplinary teams, including specialists, pharmacists, and social workers, to coordinate care, address complex patient needs, and achieve optimal health outcomes.
- Provide data-driven, condition-specific specialty visits to patients as needed
- Manage the health outcomes of an empaneled patient population
- Engage with and appropriately utilize population health data to drive superior patient outcomes
- Attend ongoing educational opportunities
- Commute to assigned assisted living communities to provide care at home
- Occasional travel to Denver market for initial and ongoing training
Physician Benefits:
- Scheduled Days: Monday - Friday (3, 4, or 5 days)

Physician Qualifications:
- Appropriate Licensure in the state of Texas
- Bilingual preferred but not required
