Case Manager

Job Summary
The Case Manager plays a vital role in supporting members with complex medical, behavioral health, and social needs. This position involves assessing members needs, developing individualized care plans, coordinating services, and ensuring members have access to housing, healthcare, behavioral health, and community resources.
Responsibilities
- Conduct outreach, engagement, and enrollment of eligible members into ECM and Community Supports programs.
- Complete intake assessments, individualized care plans, and progress documentation in a timely and accurate manner.
- Coordinate services across healthcare providers, housing agencies, and community-based partners.
- Advocate for members to ensure access to quality care and essential supports.
- Provide ongoing case management, including regular check-ins, goal tracking, and linkage to resources.
- Maintain up-to-date and compliant documentation in the care management system.
- Participate in team meetings, trainings, and case conferences.
- Uphold trauma-informed, culturally sensitive, and client-centered practices.
Experience
- Bachelor's degree in Social Work, Psychology, Human Services, Public Health, or related field (equivalent experience considered).
- Minimum of 2 years of experience providing case management, care coordination, or related social services.
- Strong communication, problem-solving, and organizational skills.
- Knowledge of Medi-Cal, CalAIM ECM and Community Supports, and local community resources preferred.
- Proficient in Microsoft Office and electronic documentation systems.
- Valid California driver's license and reliable transportation required.

Base Pay: $ $25.00 per hour
Expected hours: 40 per week
