If this sounds like how you envision your practice, Integrated Care and Consultation is seeking a part-time Family Nurse Practitioner to join our growing team.
This role will work closely with an interdisciplinary team to provide our clients with a compassionate and integrative approach to care.
The Family Nurse Practitioner will:
Provide health promotion and disease prevention services
Perform health assessments and physical exams
Order, perform, and evaluate diagnostic tests
Diagnose acute, chronic, and complex health problems
Prescribe medications and non-pharmacological treatments
Develop and manage treatment plans
Follow up and evaluate patient status
Maintain patient records
Educating patients on preventative care and disease management.
Collaborate with other healthcare providers to ensure safe and effective medication management.
Patient, family, and community education and counseling
Coordinate patient care and make referrals as needed
Formulating treatment plans for acute and chronic diseases.
Ordering and analyzing diagnostic tests.
Managing overall patient care.
REQUIREMENTS:
Must have at least 3 years of experience in a primary care setting.
Master of science in nursing (MSN).
SD CNP license.
DEA license.
Must have graduated from a nonprofit university.
Excellent interpersonal and communication skills.
Professional organization memberships.
Team player.
