Position Overview:
We are currently seeking a licensed and experienced Registered Nurse (RN) Case Manager to join our caring and dedicated team. This role is crucial to ensuring patients and their families receive high-quality, compassionate care tailored to their unique needs. Responsibilities:
Conduct thorough assessments of patients' physical, emotional, and psychosocial needs.
Develop and implement individualized care plans in collaboration with the interdisciplinary team.
Provide skilled nursing care, including symptom and medication management.
Coordinate resources, referrals, and care services for patients and families.
Offer education and emotional support to patients and their loved ones regarding end-of-life care and planning.
Maintain accurate and timely clinical documentation in accordance with hospice policies, Medicare/Medicaid guidelines, and applicable regulations.
Ensure compliance with HIPAA, state/federal healthcare laws, and organizational procedures.
Supervise and provide direction to Licensed Vocational Nurses (LVNs) and Home Health Aides (HCAs) as needed.
Participate in interdisciplinary case conferences, quality assurance activities, and continuing education.
Participate in scheduled home visits and on-call rotation to ensure 24/7 patient support.
Qualifications:
Graduate of an accredited school of nursing.
Current and valid RN license in the state of New Mexico.
At least 1 year of nursing experience; hospice or end-of-life care preferred.
Strong critical thinking, organizational, and assessment skills.
Compassionate and empathetic approach to patient care.
Excellent verbal and written communication skills.
Valid driver's license and current automobile liability insurance.
Reliable transportation for patient visits in home and community settings.
