Admissions Coordinator I

The Admissions Coordinator I is responsible for the coordination of patient admissions alongside the Admissions Supervisor in conjunction with the Admissions Director, Community Liaisons, and the Admissions RNs. The Admissions Coordinator I will ensure that all patient information is accurately entered into the patient's medical record, billing files and the daily/weekly CRM activity report.

Requirements:
Specific to Job Position:
- Acts as liaison between Admissions Director and Admissions RNs by ensuring that incoming referral information is disseminated to the correct parties and that all patient information is correctly verified and added to the patient's medical record and referral report on the CRM
- Communicates with patient, patient family or representative to provide information on the hospice benefit and ensure accurate information is given
- Appropriately educates patient, patient family or representative about hospice services, assessing needs and preferences and referring patient for services needed and desired
- Ensures that admission paperwork has been completed and is scanned into the patient's hospice medical record
- Verifies insurance eligibility and prior authorizations
- Maintains schedule of Admissions RN to ensure that all patient assessments are given sufficient time to address patient needs, questions, and concerns
- Communicates findings from Admission Director and Admissions RNs to appropriate Community Liaisons so that they can report the outcome of the patient's assessment to their referral source
- Works with Admissions Director, Admissions RNs, and Patient Care Coordinators to ensure correct information is added to the patient's hospice medical record after admission
- Completes daily and weekly activity report to communicate progress on referrals, rejected referrals, admissions, and pending admissions
- Ensures that all regulatory standards are met, including State, Federal, CHAP and other regulatory bodies
- Completes all communication and documentation accurately either electronically or in legible writing
- Coordinates patient assignments with department supervisors upon admission
- Sends out admissions texts promptly after patient admission has been completed and approved
- Sends transfer notices to transferring hospice agencies when patient has prior hospice benefits.
- Calls hospice agency to ensure transfer paperwork was received and documents conversation in patient's medical record
- Supports the philosophy, objectives, and mission of ProCare Hospice of Nevada
- Works in conjunction with the billing team
- Maintains HIPAA standards at all times
- Works well both independently and within the Interdisciplinary Team Structure
- Promotes a team approach through cooperation, mutual respect, flexibility, and dependability

General:
- Adheres to patient rights, abuse reporting and confidentiality policies
- Fosters and maintains an effective working relationship between peers, professional staff, and other personnel within the community
- Has a solid understanding of basic hospice regulations, levels of care and admission criteria
- Stays up to date on both Federal, State and all mandatory agency and regulatory requirements. Promptly educates referral sources to changes in the regulations when necessary
- Gathers data on patient referrals and admissions to create the patient's hospice medical record including patient information, insurance information, history and physical or pertinent patient medical information
- Understands/teaches basic principles of hospice and palliative care within the agency scope of practice
- Attends scheduled staff meetings and or in-service training
- Adheres to the departmental dress code requirements related to personal grooming and attire
- Participates in Performance Improvement activities as requested
- Demonstrates reliability by making every attempt to report to work on time; follows all policies related to attendance
- Demonstrates ability to problem-solve and make decisions by assessing situations, considering alternatives, seeking guidance and direction as necessary. Utilizes proper chain of command
- Demonstrates effective time management skills by coordinating activities to achieve maximum productivity and efficiency; is willing to adjust or revise work schedules as requested
- Assumes responsibility for on-going personal development and continuing education
- Completes assigned projects within specified time frames
- Seeks guidance from higher authority as needed

Physical/Safety Requirements:
- Must have the strength and endurance required for standing, walking, lifting, bending, and pushing on a consistent basis between 10 and 30 lbs.
- Utilizes appropriate durable medical equipment as needed in a safe manner
- Uses appropriate body mechanics as needed for personal and patient care
- Category 1 for potential exposure to blood/body fluids. (Requires the performance of procedures or other tasks in the work routine that involve an inherent potential for mucous membrane or skin contact with blood, body fluids or tissues, or a potential for spills or splashes of them. Protective measures should be used while engaged in Category 1 tasks with exposure potential.)
- Utilizes appropriate PPE as needed
- Follow the designated plan of action in the event of a fire or other emergency/disaster
- While performing the duties of this job, the employee is regularly exposed to fumes or airborne particles, work- related stresses, and driving hazards.
- The individual may be required to drive to meetings with clients or health care professionals, and patients' home
- The noise level in the work environment is usually minimal

Qualifications and Experience:
- One year experience in hospice, home health or geriatric care preferred, but not required
- Must be able to effectively explain the hospice benefit to the patient or patient's representative
- Successful completion of a background check pursuant to NRS and NRS 179A.100 along with a pre-employment drug screen and physical, TB testing, previous employment and reference checking, and an MVR (may include verification of education)
- Must be able to read, speak, write, and understand the English language
- Must be able to work independently, maintaining organization and accurate information
- Must be able to handle frequent interruptions, be flexible with schedule and effectively time-manage.
- Must be able to cope with emotionally charged situations encompassing, patients, families, and other caregivers
- Must be available for prescheduled evening, weekend, or holiday calls
- Must be able to effectively use all standard office equipment, including MS Office programs and internet-based programs
- Must have reliable transportation, a valid driver's license and current auto insurance
