Job Description

The Patient Navigator will work as a collaborative team member on the Navigation Services to Avoid Re-Hospitalization (NavSTAR) project, a clinical research study led by the Friends Research Institute and funded by the National Institute on Drug Abuse.

The study will compare proactive Patient Navigation services to usual care for hospital patients at Thomas Jefferson University Hospitals with opioid use disorder, a group at high risk of repeat hospitalization and negative outcomes. The Patient Navigator will work with discharged hospital patients in the community to resolve barriers to engaging in recommended follow-up medical and addiction care.

The Patient Navigator will meet with eligible study participants in the hospital and work with them for up to 3 months post-discharge, providing services including (but not limited to): helping to resolve personal barriers (e.g., lack of health insurance, transportation, etc.), providing motivational interventions to reduce ambivalence about treatment, making appointments on the patient's behalf, accompanying patients to their initial medical and substance use treatment appointments, and advocating for patients. The Patient Navigator will be required to adhere to the research protocol, work with clinical and research staff, keep detailed records of services provided, and attend regular project meetings.

The work environment includes hospital- and office-based work, as well as work in the community (including patient home visits, public places, and service agencies in the community). Reliable transportation is required.

The Patient Navigator will work closely with the NavSTAR Social Worker

Interacts with co-workers, visitors, and other staff consistent with the values of Jefferson.

· Meets with eligible study participants in the hospital for an initial patient navigation visit and in the community

as needed.

· Reviews patient chart and care plans for pertinent medical care or substance use disorder treatment information.

· Collaborates with the health care team regarding in-hospital needs and post-hospital care.

· Maintains connections with and stays up to date on community resources and programs needed by patients with

opioid use disorder.

Participates in project meetings and activities. · Applies cultural humility in interactions with patients and staff and demonstrates exceptional cultural competence with people who use drugs. · Operates with a high level of independence, as staff will often be traveling to and working without other team members onsite across Enterprise locations. · Performs other related duties as required.

Competencies:

· Strong communication and interpersonal skills, with the ability to engage with diverse populations · Proficiency in use of Microsoft Word, Excel, and email required · Proficiency in data entry and familiarity with electronic health record (EPIC) systems preferred.

Education:

· High school diploma or equivalent, associate or bachelor's degree in public Health, social work, healthcare

administration, or a related field preferred

· Certification as a Certified Recovery Specialist or Community Health Worker preferred

AND

Experience:

· Previous experience in healthcare settings, outreach work, and work with people who use drugs is strongly

preferred.

· People with lived experience of substance use/treatment/recovery are strongly encouraged to apply.

· Communicates with patients post-discharge in accordance with the intervention schedule.

· Meets with study team regularly and coordinates discharge care closely with hospital staff. In some locations,

this will be a new workflow that requires ongoing attention to achieve clinical staff buy in and partnership, that

this staff member will play a critical role in as the person working on site.

· Ensures organized and secure record-keeping and timely data entry. Documents appropriate data from in-

hospital and community-based service contacts according to project-specific requirements.
