  • Responsible for the evaluation, diagnosis, prescribing, treatment, and follow-up of patients.
  • Thoroughly documents in patients' EHR chart in compliance with company policy and Medicare standards.
  • Maintains ongoing patient follow-up of the patient's clinical information and documentation.
  • Performs assessment/data collection in an ongoing systematic manner, focusing on the holistic assessment including physiologic, cultural, and cognitive status.
  • Consults with referring physicians and other clinicians to discuss clinical protocols, patient needs, assessment findings, and obtain treatment notes.
  • Interfaces daily with the medical director, patient coordinators, and technicians.
  • Oversees the treatment of patients and provides education to the staff, including appropriate prescriptive practice to achieve optimal clinical outcomes for the patient.
  • Provides appropriate and timely documentation of the treatment plan and patient progress.
  • Manages software to assist with patients and provide new patients' intake process.
  • Ensures that records are detailed and complete, complying with related standards and regulations.
  • Initiates as well as collaboratively shapes the patient and staff experience to maximize both individual and clinic-wide improvements as needed to achieve optimal healing for the patients.
  • Builds ongoing relationships and provides education to the healthcare team members, local physicians, and medical and community organizations.
  • Demonstrates proficiency in the use of computers and telecommunication modalities in documenting, tracking, and conveying information.
  • Performs other related duties as necessary.

Job Types: Full-time, Part-time

Pay: From $85.00 per hour

Expected hours: 24 – 40 per week
