Overview

We are seeking a dedicated Case Manager to join our team. The Case Manager will monitor and oversee a caseload of children receiving Early Intervention (EI) services. This role requires strong organizational skills, attention to detail, and the ability to work effectively in a fast-paced environment.

Duties:

-Maintain accurate and up-to-date records and documentation in accordance with DOH EIP regulations.

-Coordinate, facilitate, and monitor the delivery of services required in the IFSP to ensure that the services are provided in a timely manner and meet all compliance indicators and regulations.

-Monitor newly approved service authorizations and provide therapists with necessary information and documentation.

-Support therapists and families and ensure that Early Intervention services are provided in accordance with DOH EIP rules and regulations.

-Respond to phone/ email inquiries from therapists, service coordinators, parents and office staff.

-Communicate with pediatricians and medical personnel to obtain medical forms/ prescriptions.

-Update the company's database and ensure it contains accurate information.

Skills:

-Excellent verbal, written, and interpersonal communication skills

-Ability to work independently as well as part of a team

-Strong organizational, time-management, and analytical skills
