Eisner Health is a Federally Qualified Health Center (FQHC) providing culturally competent, full life-cycle medical care, prenatal care, women's health services, dental care, behavioral health care, optometry, dermatology, case management/care coordination, supplemental services (patient benefits and enrollment, outreach), and an on-site pharmacy and lab. We are the provider of choice for more than 40,000 children, adults, and older adults in Los Angeles County each year.
Position Summary:
Under the direction of the Quality Improvement (QI) Manager, the QI Specialist facilitates quality improvement projects for individual departments and the organization. The scope of responsibility encompasses all settings of the organization, both clinical and operational. The QI Specialist is responsible for analyzing and reporting findings, as well as developing tools to measure improvement. The QI Specialist has an understanding of the quality improvement process and techniques required to facilitate quality improvement projects.
The QI Specialist supports the planning, implementation, and evaluation of organizational quality initiatives led by the QI Manager and Director of Quality Operations. The role focuses on data integrity, workflow optimization, care gap closure, regulatory reporting support, and frontline staff training to improve clinical quality, compliance, and patient experience.
The QI Specialist collaborates with clinical teams, care coordinators, population health staff, and informatics to operationalize performance improvement activities across Eisner Health in alignment with HRSA, CMS, NCQA-PCMH, Medi-Cal MCP, and health plan requirements. This role contributes directly to achieving organizational performance goals by translating measure logic into actionable workflows and supporting data-driven decision-making.
Essential Duties:
Quality Program Support & Regulatory Alignment
- Supports execution of the organizational Quality Improvement Program in alignment with HRSA, UDS, HEDIS, CMS, NCQA-PCMH, PHMI, and Medi-Cal MCP reporting requirements.
- Assists in preparing documentation and evidence for audits, NCQA-PCMH recognition, and HRSA OSV readiness.
- Acts as the measure lead for assigned CQMs by monitoring performance trends, providing staff training and onboarding support, and communicating updates regarding QI initiatives to ensure alignment across departments.
- Maintains working knowledge of quality measure logic and updates related to changing regulatory standards.
- Maintains working knowledge of EHR and population health management functionality for data collection, report building, and data analysis.
- Maintains working knowledge of user registration and clinical workflows.
- Assists in preparing materials for QI Committee, workgroups, and internal QI communications.
- Attends external QI collaboratives, learning sessions, and professional conferences, shares key findings and best practices, and communicates relevant updates and improvement opportunities to leadership and cross-functional teams.
Data Integrity, Reporting, & Care Gap Analytics
- Generates care gap lists and registry reports, using EHR, payer portals, and population health platforms.
- Conducts data validation, resolves documentation inconsistencies, and escalates issues affecting measure performance to department leadership.
- Transforms raw data into clear tables, visuals, and dashboards for operational and clinical decision-making, and reconciles data across platforms (e.g., EHR, population health tools, Excel lookups, pivot tables, formulas) to ensure accuracy and consistency.
- Prepares performance summaries to support department leadership reporting and discussion with the QI Committee and other leadership teams.
Performance Improvement, Workflow Optimization & Clinic Engagement
- Participates in PDSA cycles, root cause analyses, and workflow redesign activities.
- Communicates performance progress, barriers, and key updates to the QI Core Team, departmental QI workgroups, and organizational leadership as appropriate.
- Creates workflow maps and process flow charts using platforms such as Visio, supporting standardization and communication of QI workflows across teams.
- Develops and maintains tip sheets, workflow guides, job aids, and standing-order implementation tools.
- Supports standardization of pre-visit planning (PVP) and point-of-care gap closure workflows. Provides in-service training (including at-the-elbow support) to clinical and operational staff on documentation standards and care gap workflows.
- Assists with implementing new workflows and tracks adoption, utilization, and performance outcomes under the direction of department leadership.
Patient Engagement
- Facilitates patient outreach strategies such as text campaigns, portal notifications, and telephone outreach to support care gap closure and appointment scheduling.
- Supports patient experience and satisfaction surveying activities, including follow-up summaries to QI Manager.
Requirements and Qualifications:
- Bilingual required (Spanish).
- Bachelor's degree in a relevant field, or comparable experience and training.
- Preferably one year of experience conducting PDSAs, providing project coordination and oversight, health education, or research.
- Strong analytical, organizational, and planning skills, with the ability to manage multiple projects.
- Demonstrated skills in data collection with strong attention to detail.
- Experience preparing visual presentations using multimedia tools (e.g., Canva, PowerPoint)
- Preferred experience working with spreadsheets and preparing pivot tables, charts, and graphs.
- Experience using EHR and population health platforms for data collection, reporting, or care gap management is preferred.
- Experience creating workflow maps or process flow charts and preparing tip sheets, job aids, or workflow guides is preferred.
