Job Description

Specific duties include the following but are not limited to:

  • Process, monitor and review all reportable and non-reportable incidents in conformance with Part 624 of 14NYCRR.
  • Conduct investigations as assigned by NYS Justice Center and Incident Management Coordinator.
  • On-call 24 hours/7 days week for reporting and investigation of urgent agency incidents.
  • Assist in the staff training process regarding incident identification and reporting.
  • Interface with Agency personnel and community/service providers.
  • Attend monthly Incident Review Committee meetings. Record meeting minutes ensuring adequate description and accuracy. Generate meeting minutes/reports in a timely manner.
  • Coordinate with Quality Assurance staff to ensure on-going compliance with federal and state incident management requirements.
  • Attend OPWDD facility audits when requested.
  • Develop compliance protocol and administrative guidelines relative to in the incident management process.
  • Update the incident management data base, as necessary.
  • Assist Quality Assurance staff in implementing the internal QA process, as required.
  • Perform additional duties as required.

Requirements:

  • Bachelor's degree required.
  • Ability to multi-task. Must have a positive, pleasant disposition and exhibit meticulous attention to detail.
  • Must be able to maintain strict confidentiality.
  • Must have a valid drivers' license.
  • Excellent computer skills
