Job Summary:
Under the direction of the Vice President of Support Services, the Manager of Rehabilitation and Life Enrichment Services provides strategic and operational leadership to ensure the delivery of high-quality, person-centered rehabilitation and enrichment programming across all PACE-RI settings. This role requires a creative and resourceful leader who can design and implement engaging, clinically meaningful programs that enhance participants' physical, emotional, and social well-being. The manager fosters collaboration and accountability among interdisciplinary team members and staff, maintaining composure and sound judgment in complex or high-pressure situations. With exceptional communication and presentation skills, this leader drives engagement, aligns services with care plan goals, and promotes professional growth across departments. Guided by a data-informed and quality-improvement mindset, the manager ensures that all services meet regulatory standards, support optimal participant outcomes, and reflect the mission and values of PACE-RI.
Job Duties/Responsibilities:
- Responsible for the effective coordination of rehabilitative and life enrichment services both in the home setting and in PACE centers, the Manager of Rehabilitation and Life Enrichment Services oversees staff performance, utilization of services, serves as Interdisciplinary team member when required, coordinates services outlined in the care plan and along with the IDT, assists in determining the need for continued rehab and life enrichment services.
- Provides utilization management and review of skilled placements and gathers information for the IDT regarding continued necessity of such placements to make informed, best practice decisions around medically necessary care.
- Monitors and maintains relationships with vendors around custodial, long-term placements of participants in order assess quality of care, utilization of services in the long-term care setting and provides information and make recommendations to the IDT.
- Assists in the selection and orientation of employees. Maintains appropriate level of training and orientation of staff members and provides on-going training as needed in department as identified.
- Assists with oversight, implementation, and adherence to policies, procedures, and standards for all services in consultation with other management staff.
- Communicates with participants, families, contracted providers, and the public regarding issues related to the routine operation of wellness services, resolves all grievances and complaints received related to department.
- Oversees the provision and quality of life enrichment and rehabilitative services and monitors for adherence to regulations on an on-going basis
- Serves on the Clinical Management Team to ensure quality clinical services and care plan integrity for participants of PACE.
- Hires, supervises, and trains Life Enrichment (LE) staff so they implement the plan effectively which results in consistent, inspiring, and joyful programming. Completes annual evaluations.
- Works with the LE Leads to ensure the creation of monthly calendars that detail life enrichment offerings. Additionally, ensures choices available for participants respond to a wide range of clinical needs and appropriately incorporates care plan goals.
- Ensures the development and implementation of a comprehensive activities program to help meet the physical, social, intellectual, artistic, and spiritual needs of the participants.
- Works collaboratively with the Day Center Directors on implementation of programs in a way that encourages and fosters teamwork and positive outcomes
- Conducts qualitative and quantitative medical record audits to ensure proper delivery and allocation of services
- Works with IDT to develop and implement protocols to respond to chronic illness and prevent health crisis/hospitalization.
- Attends and participates in staff meetings, in-services, projects, and committees as assigned.
- Adheres to and supports the facilities' policies, practices, and procedures.
- Accepts assigned duties in a cooperative manner and perform all other related duties as assigned.
- Develops and will participate in Administrative on-call duties as required as well as opening and/or closing of the PACE center.
- Participates in periodic assessments and coordinating 24-hour care delivery.
- Maintains professional licensure and competencies necessary for direct care services provided, if applicable.
- Create sand implements departmental audit system to identify areas of non-compliance and opportunities for quality improvement; use data to inform decision making and action steps.
- Collaborates with and supports Canopy Care Solutions initiatives, integrating them where opportunities arise.
- Required to work at all PACE-RI sites as necessary as well as in the community as needed.
- Performs other related duties as required and assigned.
Required Skills & Abilities:

Education Requirements:
Associate's Degree, Required
Bachelor's Degree, Preferred
Physical Requirements:
Physical ability to regularly stoop, bend, kneel, lift, stand, walk, stretch and reach for extended periods of time.
Must be able to regularly lift objects up to 25 pounds and at times up to 35 pounds.
Must be able to assist a lb. participant, using proper body mechanics.
Must have hearing and vision abilities within normal range (corrected) to drive safely and/or to observe and communicate with participants.
Must be able to navigate various departments of the organization's physical premises as well as related community care settings.
Must be able to tolerate conditions typically associated within a medical office and/or home care setting including potential exposure to bloodborne pathogens and infectious diseases.
Must be able to complete assignments in variable weather conditions and ever-changing and sometimes allergenic environments in participant homes, including extreme heat, humidity, dust, smoke, pet contamination, and cleaning supplies.
Experience:
3 to 5 Years of Direct Service Work, Required
3 Years of Experience in a Medical or Geriatric Setting, Preferred
2 to 5 Years Staff Management Experience, Required
1 to 3 Years of Program Management Experience, Preferred
License & Certification Requirements:
RI Licensure in one of the following disciplines (OT, PT, Speech Therapy, COTA, PTA, or Recreation Therapy), Highly Preferred but not Required.

CPR Certification
Driver's License & access to reliable transportation: community-based travel required
