ESSENTIAL FUNCTIONS:
- Assess family safety and reporting any concerns regarding the safety of children to supervisor immediately.
- Accept referrals from the referral agency and schedule families for services.
- Coordinate and facilitate meetings for each family accepted; which will include the family, referral source, and other family supports.
- Provide services to each referred family and completes appropriate parenting training curriculum.
- Collaborate with referral source to develop a plan in meeting program requirements and services provided for each client.
- Provide required documentation in accordance with program standards and contract obligations.
- Meet with referral source personnel regularly to provide ongoing reports of the family's progress.
- Maintain appropriate professional boundaries with clients and families.
- Assess family's strengths and needs.
- Schedule and coordinate visits and transportation of clients and families.
- Meet with families to discuss goals, strengths, and opportunities for improvement.
- Prioritize and target identified risk factors and measure progress for the use during planning meetings.
- Input information into the electronic databases.
JOB QUALIFICATIONS:
- Bachelor's degree in Social Work or related field.
